【摘要】:目的觀察與比較七氟烷和丙泊酚全麻誘導(dǎo)預(yù)防芬太尼誘發(fā)嗆咳反應(yīng)的效果及對(duì)誘導(dǎo)期血流動(dòng)力學(xué)的影響。 方法選取ASAI-II級(jí),年齡20-60歲,體重40-80kg的擇期手術(shù)患者60例,隨機(jī)分為七氟烷誘導(dǎo)組(S組)和丙泊酚誘導(dǎo)組(P組),每組30例。S組用潮氣量法行吸入麻醉誘導(dǎo)。P組用丙泊酚靶控輸注(TCI)行靜脈麻醉誘導(dǎo)。當(dāng)兩組病人意識(shí)喪失,呼之不應(yīng),睫毛反射消失,腦電雙頻指數(shù)(BIS)值介于40-60時(shí),均從外周肘靜脈單向靜脈注射芬太尼原液5ug/kg,,注射時(shí)間為5s。觀察靜注芬太尼后1min內(nèi)嗆咳反應(yīng)發(fā)生的有無(wú)及其嚴(yán)重程度,嚴(yán)重程度評(píng)分標(biāo)準(zhǔn)為:無(wú)嗆咳(0次),輕度嗆咳(1-2次),中度嗆咳(3-4次),重度嗆咳(≥5次)。觀察并記錄麻醉誘導(dǎo)前(T0),靜注芬太尼前(T1),靜注芬太尼后1分鐘(T2),及插管后1分鐘(T3)的血壓(BP)、心率(HR)值來(lái)比較兩種麻醉誘導(dǎo)方法對(duì)誘導(dǎo)期血流動(dòng)力學(xué)的影響。 結(jié)果1:兩組患者在年齡、性別、體重、基礎(chǔ)血壓和基礎(chǔ)心率等方面無(wú)統(tǒng)計(jì)學(xué)差異(P㧐0.05)。 2:S組嗆咳反應(yīng)的發(fā)生率為6.7%,P組嗆咳反應(yīng)的發(fā)生率為43.3%(P<0.01)。S組嗆咳反應(yīng)的嚴(yán)重程度明顯低于P組(P<0.01)。 3:兩組患者血流動(dòng)力學(xué)變化比較和T0比較,S組血壓在T1、T2下降,在T3血壓上升(P<0.05),P組血壓在T1、T2下降,在T3血壓上升(P<0.05);S組心率在T1、T2變化無(wú)統(tǒng)計(jì)學(xué)差異,在T3增快(P<0.05),P組心率在T1、T2、T3增快(P<0.05)。組間比較, P組血壓在T1低于S組,P組心率在T1、T2、T3高于S組,具有統(tǒng)計(jì)學(xué)差異(P<0.05)。 結(jié)論七氟烷誘導(dǎo)可顯著降低芬太尼誘發(fā)嗆咳反應(yīng)的發(fā)生率及嚴(yán)重程度,能更好地維持誘導(dǎo)期血流動(dòng)力學(xué)的穩(wěn)定。
[Abstract]:Objective to observe and compare the effects of sevoflurane and propofol on preventing cough induced by fentanyl and hemodynamics during induction. Methods Sixty ASAI-II patients aged 20-60 years and weighing 40-80kg were randomly divided into sevoflurane induced group (S) and propofol induced group (P). 30 cases in group S were induced by inhalation anesthesia with tidal volume method. Group P was induced by intravenous anesthesia with target controlled infusion of propofol (TCI). When the patients in both groups lost consciousness and exhaled, the eyelash reflex disappeared. When the BIS value of EEG was between 40 and 60, fentanyl was injected into the cubital vein in one direction (5ugr / kg). The time of injection was 5 s. The severity of cough reaction within 1 min after intravenous injection of fentanyl was observed. The score of severity was 0 times without choking cough and 1-2 times with mild cough).
