【摘要】：目的對比分析小兒七氟烷麻醉中不同劑量瑞芬太尼持續(xù)輸注下自主呼吸情況。方法選擇從2012年12月至2014年12月在本院進行斜視矯正手術(shù)治療的患兒120例,以30例為1組,分為甲、乙、丙、丁四組,甲、乙、丙三組持續(xù)輸注瑞芬太尼,劑量分別是0.03μg/kg/min、0.06μg/kg/min、0.09μg/kg/min。丁組持續(xù)輸注0.9%生理鹽水。觀察并對比兩組的應(yīng)用效果。結(jié)果組間比較:甲、乙、丁三組在B時RR達(dá)到最低,隨后逐漸上升,在E時有所下降;丙組RR隨時間推移逐漸降低,在E時有所升高,比較差異均具有統(tǒng)計學(xué)意義(P0.05)。組間比較,四組A時、B時的RR無顯著差異(P0.05):與丁組相比,甲乙丙三組CDE的RR更低(P0.05),且丙組低于甲組和乙組(P0.05),甲組和乙組無顯著差異(P0.05)。組內(nèi)比較,甲、乙、丁的VT先逐漸降低,D時升高,E時降低,比較差異均具有統(tǒng)計學(xué)意義(P0.05);丙組各時點VT無顯著差異(P0.05);組間比較,四組各時點的VT無無顯著差異(P0.05)。結(jié)論在小兒七氟烷麻醉中以0.03-0.09μg/kg/min速率持續(xù)輸注瑞芬太尼可抑制患兒的自主呼吸,其中0.03--0.06μg/kg/min輸注速率可在滿足鎮(zhèn)痛需要的基礎(chǔ)上盡可能保留患兒的自主呼吸,應(yīng)根據(jù)手術(shù)需要進行調(diào)整。
[Abstract]:Objective to compare and analyze the spontaneous respiration during sevoflurane anesthesia in children with different doses of remifentanil continuous infusion. Methods from December 2012 to December 2014, we performed strabismus correction surgery in our hospital. 120 cases were children. 30 cases were divided into three groups: group A, B, C, D, A, B, C, D, A, B, C and C were continuously infused with remifentanil at a dose of 0.03 渭 g / kg / kg / min, 0.06 渭 g / kg / min, respectively. 0.09 渭 g / kg 路min 路min group was continuously infused with 0.9% normal saline. Conclusion continuous infusion of remifentanil at a rate of 0.03-0.09 渭 g / kg / min during sevoflurane anesthesia in children can inhibit the spontaneous respiration of children. The infusion rate of 0.03-0.06 渭 g / kg / min can keep the patient's spontaneous respiration on the basis of satisfying the need of analgesia, and should be adjusted according to the need of operation.
