無(wú)錫市2010-2012年擴(kuò)大流感監(jiān)測(cè)結(jié)果分析

【摘要】:目的分析2010-2012年無(wú)錫市流感監(jiān)測(cè)結(jié)果,掌握其流行規(guī)律,病毒型別及其活動(dòng)水平,為防控流感提供科學(xué)依據(jù)。方法分析2010-2012年無(wú)錫市原有國(guó)家級(jí)監(jiān)測(cè)哨點(diǎn)流感監(jiān)測(cè)結(jié)果以及2012年縣級(jí)流感監(jiān)測(cè)結(jié)果。流感監(jiān)測(cè)內(nèi)容包括流行病學(xué)和病原學(xué)兩方面,采集哨點(diǎn)醫(yī)院部分ILI咽拭子標(biāo)本,用實(shí)時(shí)熒光聚合酶鏈反應(yīng)(RT-PCR)分子生物學(xué)方法和雞胚或狗腎細(xì)胞(MDCK)培養(yǎng)方法進(jìn)行病毒分離,采用血凝抑制實(shí)驗(yàn)對(duì)分離的流感毒株進(jìn)行型別鑒定。結(jié)果 2010-2012年2所國(guó)家級(jí)哨點(diǎn)醫(yī)院門(mén)診就診者共計(jì)1 354 671例,其中ILI 28 919例,占2.13%,ILI%每年出現(xiàn)冬春季和夏季兩個(gè)流行高峰,70.30%的ILI集中在5歲以下年齡組,流感核酸陽(yáng)性率與ILI%相關(guān)性無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),部分高峰月陽(yáng)性率與ILI%略有偏移,或ILI%未見(jiàn)顯著高峰值。2012年江陰市2家縣級(jí)流感監(jiān)測(cè)哨點(diǎn)醫(yī)院流感毒株陽(yáng)性率(72.73%)與ILI%(27.47%)均在30周達(dá)到高峰值,兩者呈正相關(guān)(相關(guān)系數(shù)為0.738,P0.01)。采集鼻咽拭子3 720份,檢出流感病毒566株,陽(yáng)性率為15.22%,其中A(H3)型、甲型H1N1型和B型分別占37.81%、11.66%和50.53%。15~24歲年齡組流感病毒陽(yáng)性率及B型、甲型H1N1流感陽(yáng)性率最高,60歲以上老人(H3)型陽(yáng)性率最高。結(jié)論經(jīng)過(guò)甲型H1N1流感大流行后,無(wú)錫市2010-2012年度流感處于季節(jié)性常態(tài)流行,每年ILI和病毒陽(yáng)性率均有兩個(gè)高峰,即冬春季和夏季;優(yōu)勢(shì)菌株之間的交替有一定規(guī)律;流感病毒活動(dòng)與ILI之間有一定相關(guān)性,基層局部地區(qū)ILI監(jiān)測(cè)數(shù)據(jù)靈敏度可能更高,ILI%與陽(yáng)性率吻合度更高。
[Abstract]:Objective to analyze the surveillance results of influenza in Wuxi city in 2010 / 2012, to master its epidemic law, virus type and activity level, and to provide scientific basis for influenza prevention and control. Methods the results of sentinel influenza surveillance in Wuxi in 2012 and county influenza surveillance in 2012 were analyzed. Influenza surveillance included epidemiological and etiological aspects. Some ILI throat swabs were collected from sentinel hospitals. The virus was isolated by real-time fluorescence polymerase chain reaction (RT-PCR) molecular biology and (MDCK) culture of chicken embryo or dog kidney cells. The isolated influenza strains were identified by hemagglutination inhibition test. Results from 2010 to 2012, there were 1 354671 outpatients in two national sentinel hospitals, including 28919 cases of ILI (2.13%). ILI% had two epidemic peaks in winter, spring and summer every year, and 70.30% of ILI were concentrated in the age group under 5 years old. There was no significant correlation between the positive rate of influenza nucleic acid and ILI% (P 0.05), and the monthly positive rate of some peaks was slightly different from that of ILI%. There was no significant peak value of ILI%. In 2012, the positive rate of influenza strains (72.73%) and ILI% (27.47%) in two county-level influenza surveillance sentinel hospitals in Jiangyin City reached a high peak at 30 weeks, and there was a positive correlation between them (correlation coefficient 0.738, P 0.01). 566 strains of influenza virus were detected in 3720 nasopharyngeal swabs, the positive rate was 15.22%.
