上海市某醫(yī)院感染死亡患者相關(guān)因素分析

【摘要】:目的了解上海某三級綜合性醫(yī)院的醫(yī)院感染死亡患者相關(guān)危險因素及其成本效益,加強(qiáng)醫(yī)院感染的相關(guān)危險因素的動態(tài)監(jiān)測,并及時采取相應(yīng)的醫(yī)院感染預(yù)防措施,使醫(yī)院感染管理決策更具科學(xué)性、合理性。方法通過醫(yī)院感染監(jiān)測系統(tǒng)和醫(yī)師工作站獲取該醫(yī)院2012-2013年醫(yī)院感染死亡病例相關(guān)數(shù)據(jù),運(yùn)用Excel2013以及SPSS18.0對相關(guān)數(shù)據(jù)進(jìn)行整理、分析。結(jié)果兩年發(fā)生醫(yī)院感染患者共1 506例,醫(yī)院感染死亡63例,病死率為4.2%;醫(yī)院感染患者年齡(65.4±16.6)歲,60歲的老年患者死亡比例最高;住院天數(shù)為(2~2 164)d,中值為28d;醫(yī)院感染死亡患者下呼吸道感染率為54.0%;科室分布中以急救科最多,占20.6%;感染的革蘭陰性菌占62.1%;醫(yī)院感染死亡患者中住院總費(fèi)用中位值為168 174.4元。結(jié)論上海市某醫(yī)院醫(yī)院感染病死率處于較高水平,醫(yī)院感染死亡患者住院天數(shù)的延長,增加總體醫(yī)療費(fèi)用;感染部位以下呼吸道感染為主,主要病原菌為革蘭陰性菌;患者年齡大、基礎(chǔ)疾病嚴(yán)重是醫(yī)院感染的危險因素。
