院內(nèi)感染患者致病菌的流行病學(xué)調(diào)查及主要致病菌耐藥機制的初步探討

【摘要】:院內(nèi)感染是指發(fā)生在醫(yī)院內(nèi)的一切感染,包括菌血癥、肺炎、尿路感染和手術(shù)部位的感染等,醫(yī)院感染不僅影響患者的康復(fù)和增加費用,也給國家有限的衛(wèi)生資源帶來巨大的浪費,同時增加了醫(yī)護人員的壓力。隨著醫(yī)學(xué)科學(xué)的飛速發(fā)展,降低院內(nèi)感染已成為當(dāng)今研究的新課題。耐藥菌株是引發(fā)院內(nèi)感染的主要病原微生物,此類微生物經(jīng)過抗生素的多次選擇性作用,已經(jīng)形成耐藥性很強的菌株,而且長期在醫(yī)院內(nèi)流行。吉林大學(xué)第一醫(yī)院是我省最大的綜合性教學(xué)醫(yī)院,抗生素使用量大,而且大多數(shù)新藥又是首先被使用,故該院細菌的耐藥問題具有一定的代表性。為此于2003年10月至2004年10月對吉大一院主要臨床科室住院感染以及致病菌分布情況和這些科室住院感染主要致病菌的耐藥性進行了一年的調(diào)查,并利用質(zhì)粒提取試驗和質(zhì)粒消除試驗對主要致病菌的耐藥機制予以初步探討。為吉林大學(xué)第一醫(yī)院住院感染細菌的流行病學(xué)及分子流行病學(xué)研究提供第一手資料。 本實驗的主要工作包括以下幾個方面: 一、吉林大學(xué)第一醫(yī)院院內(nèi)感染流行病學(xué)研究 1、部分科室住院患者感染以及致病菌分布情況的調(diào)查 2、部分科室感染性疾病主要致病菌的耐藥性的調(diào)查 二、主要致感染性疾病細菌的耐藥機制分析 1、引起吉林大學(xué)第一醫(yī)院院內(nèi)感染的主要致病菌耐藥性R質(zhì)粒的提 取。 2、R質(zhì)粒消除實驗
[Abstract]:Nosocomial infection refers to all infections occurring in the hospital, including bacteremia, pneumonia, urinary tract infection and surgical site infection. Nosocomial infection not only affects the patient's rehabilitation and increases the cost. It also creates a huge waste of the nation's limited health resources and increases the pressure on health care workers. With the rapid development of medical science, reducing nosocomial infection has become a new research topic. Drug resistant strains are the main pathogenic microorganisms that cause nosocomial infection. After many times of selective action of antibiotics, the resistant strains have been formed and have been prevalent in hospitals for a long time. The first Hospital of Jilin University is the largest comprehensive teaching hospital in our province. The use of antibiotics is large, and most new drugs are first used, so the problem of drug resistance of bacteria in Jilin University is representative. From October 2003 to October 2004, we investigated the main clinical infection and the distribution of pathogenic bacteria and the drug resistance of the main pathogens in the first Hospital of Jida from October 2003 to October 2004. The drug resistance mechanism of main pathogenic bacteria was preliminarily discussed by plasmid extraction test and plasmid elimination test. To provide first-hand data for the epidemiology and molecular epidemiology of infection bacteria in the first Hospital of Jilin University. The main work of this experiment includes the following aspects: 1. The epidemiological study of nosocomial infection in the first Hospital of Jilin University. Investigation of infection and distribution of pathogenic bacteria among inpatients in some departments 2. Investigation of drug resistance of the main pathogens of infectious diseases in some departments. Analysis of drug resistance mechanism of bacteria causing major infectious diseases 1. Extraction of resistant R plasmids of the main pathogenic bacteria caused by nosocomial infection in the first Hospital of Jilin University. Elimination experiment of 2R plasmid
