【摘要】：目的研究血清胃蛋白酶原(Pepsinogen,PG)、胃泌素-17(Gastrin-17,G-17)和幽門螺桿菌(Helicobacter pylori,H.pylori)抗體在福建省沿海地區(qū)居民中的分布特征,探討這些血清學指標用于該省人群胃癌早期篩查的可能性。方法采用多階段分層隨機抽樣方法在長樂江田鎮(zhèn)、同安大同鎮(zhèn)和福安賽歧鎮(zhèn)選擇當?shù)鼐用駷檎{(diào)查對象,以ELISA法檢測血清PGⅠ、PGⅡ、G-17和H.pylori抗體,用中位數(shù)和率描述上述指標的分布特征,并用秩和檢驗、卡方檢驗和多重線性回歸分析方法進行統(tǒng)計學分析。結(jié)果 PGⅠ、PGⅡ、PGⅠ/Ⅱ比值(PGR)、G-17的中位數(shù)水平分別為113.75μg/L、12.0μg/L、9.89、1.4 pmol/L,H.pylori感染率為37.1%。男性PGⅠ、PGⅡ水平顯著高于女性,PGⅡ隨年齡增加而升高,有胃癌家族史居民G-17水平顯著高于無家族史者,長樂江田鎮(zhèn)H.pylori感染率顯著高于其他兩地,PGⅠ、PGⅡ、PGR和G-17水平在不同調(diào)查地區(qū)和H.pylori感染人群中的差異均有統(tǒng)計學意義。結(jié)論福建省沿海地區(qū)居民血清PGⅠ、PGⅡ、G-17和H.pylori抗體水平在不同性別、年齡、胃癌家族史和調(diào)查地區(qū)上可能存在差異。
