中國(guó)成年男性第一秒用力呼氣容積正常參考值的地理分布規(guī)律研究

【摘要】: 健康人肺功能正常參考值是醫(yī)學(xué)正常參考值的一個(gè)重要指標(biāo)。肺功能檢測(cè)是對(duì)塵肺病人進(jìn)行勞動(dòng)能力鑒定的主要依據(jù)之一,也是客觀評(píng)價(jià)接塵工人肺功能損害的主要檢測(cè)方法,而健康人第一秒用力呼氣容積正常參考值是健康人肺功能正常參考值的一個(gè)重要指標(biāo),,是常用的判斷中小氣道有無(wú)阻塞的指標(biāo)。第一秒用力呼氣容積正常參考值是對(duì)疾病進(jìn)行診斷和防治的基本依據(jù),每個(gè)地區(qū)、每個(gè)醫(yī)院都必須建立一套第一秒用力呼氣容積正常參考值標(biāo)準(zhǔn)。但由于各地地理環(huán)境不同,測(cè)定的儀器、對(duì)象、方法各異,使得各地區(qū)第一秒用力呼氣容積正常參考值不一致,因此嚴(yán)重影響了臨床診斷的準(zhǔn)確性。 本論文以國(guó)家自然科學(xué)基金資助項(xiàng)目“中國(guó)人肺功能正常參考值與地理因素的關(guān)系”(項(xiàng)目號(hào)40671005)為依托,以醫(yī)學(xué)地理學(xué)理論、肺功能理論、自然地理學(xué)理論和數(shù)理統(tǒng)計(jì)原理為依據(jù),運(yùn)用計(jì)算機(jī)手段,主要是SPSS11.5統(tǒng)計(jì)軟件、Excel數(shù)據(jù)處理軟件、地理信息系統(tǒng)制圖軟件(MapInfo),對(duì)第一秒用力呼氣容積正常參考值與地理因素關(guān)系進(jìn)行了專(zhuān)題化、定量化研究。 本論文把數(shù)據(jù)分為18-40歲中青年和40-65歲中老年兩個(gè)年齡段,依次對(duì)中青年和中老年進(jìn)行一元線性回歸分析、多元線性回歸分析、逐步回歸分析、非線性回歸分析,通過(guò)分析可以看出中青年男性第一秒用力呼氣容積和中老年男性第一秒用力呼氣容積與所選地理因素并非都具有相關(guān)性,中青年第一秒用力呼氣容積(Y_1)與年平均相對(duì)濕度(X_7)相關(guān)性極顯著,與海拔高度(X_1)、年降水量(X_3)、年日照時(shí)數(shù)(X_8)相關(guān)性顯著,中老年第一秒用力呼氣容積(Y_2)與年平均氣溫(X_2)、一月平均氣溫(X_5)、七月平均氣溫(X_6)、年平均相對(duì)濕度(X_7)之間相關(guān)性顯著,它們的相關(guān)性不是單一的,而是與地理因素的多重相關(guān),運(yùn)用SPSS 11.5統(tǒng)計(jì)軟件最后得出一系列的回歸方程。并在相關(guān)分析的基礎(chǔ)上比較得出了兩個(gè)最優(yōu)的模型: ①、18-40歲中青年健康成年男性第一秒用力呼氣容積正常參考值預(yù)測(cè)模型: Y_1=1.906+0.026~*X_7 F=11.605,P=0.000<0.01 Y_1的預(yù)測(cè)區(qū)間為[Y_1-0.519,Y_1+0.519] 0.519為1.96個(gè)剩余標(biāo)準(zhǔn)差 ②、40-65歲中老年健康成年男性第一秒用力呼氣容積正常參考值預(yù)測(cè)模型: Y_2=0.958+0.15~*X_4+0.036~*X_5+0.07~*X_6 F=5.894,P=0.009<0.01 Y_2的預(yù)測(cè)區(qū)間為[Y_2-0.366,Y_2+0.366] 0.366為1.96個(gè)剩余標(biāo)準(zhǔn)差 選取8個(gè)具有代表性的地區(qū),應(yīng)用以上模型對(duì)該地區(qū)中青年男性和中老年男性正常參考值進(jìn)行預(yù)測(cè),并將預(yù)測(cè)值與實(shí)測(cè)值進(jìn)行比較,發(fā)現(xiàn)中青年男性和中老年男性的預(yù)測(cè)值與實(shí)測(cè)值高度一致,無(wú)顯著性差異,從而說(shuō)明以上一系列回歸模型可以預(yù)測(cè)某一地區(qū)中青年男性和中老年男性正常參考值,為從地理因素入手計(jì)算某一地區(qū)中青年男性和中老年男性第一秒用力呼氣容積正常參考值提供了簡(jiǎn)便的方法。 通過(guò)研究中國(guó)成年男性第一秒用力呼氣容積正常參考值與地理因素的關(guān)系,揭示出了中國(guó)成年男性第一秒用力呼氣容積正常參考值的地理分布規(guī)律,對(duì)制定中國(guó)成年男性第一秒用力呼氣容積正常參考值的標(biāo)準(zhǔn)提供了科學(xué)依據(jù),將促進(jìn)醫(yī)學(xué)更快的發(fā)展,本論文的研究方法和研究成果將豐富醫(yī)學(xué)地理學(xué)的內(nèi)容。研究具有重要的現(xiàn)實(shí)意義。
[Abstract]:Normal reference value of lung function of healthy people is an important index of normal medical reference value. Lung function test is one of the main basis for identifying the ability of pneumoconiosis patients' ability to work. It is also the main method to evaluate the lung function damage of the exposed workers, and the normal reference value of the healthy people for the first second of the force is the healthy person's lung function. An important index of normal reference value is a common indicator of small and medium airway obstruction. The normal reference value of the first second forced expiratory volume is the basic basis for the diagnosis and prevention of the disease. Each area, each hospital must establish a set of normal reference values for the first second forced expiratory volume. The environment is different, the instruments, objects and methods are different. The normal reference value of the forced expiratory volume in the first second of each region is inconsistent, so the accuracy of the clinical diagnosis is seriously affected.
This paper is based on the "relationship between normal reference value of Chinese lung function and geographical factors" (Project No. 40671005) funded by the National Natural Science Foundation of China, based on the theory of medical geography, lung function theory, the theory of natural geography and the principle of mathematical statistics, and using the means of counting computer, mainly SPSS11.5 statistics software, Excel data The processing software, the geographic information system mapping software (MapInfo), has been thematic and quantitative research on the relationship between the normal reference value of the forced expiratory volume of the first second and the geographical factors.
In this paper, the data are divided into two age groups of 18-40 years old and 40-65 years old and middle aged and old age. The linear regression analysis, multiple linear regression analysis, stepwise regression analysis, nonlinear regression analysis are carried out in turn to middle and young people and middle and old age, and the first second forced expiratory volume and the first second of middle-aged and old men can be seen through analysis. The forced expiratory volume was not correlated with the selected geographical factors. The correlation between the first second forced expiratory volume (Y_1) and the annual average relative humidity (X_7) was very significant. The correlation was significant with the altitude (X_1), the annual precipitation (X_3), the annual sunshine hours (X_8), the first second forced expiratory volume (Y_2) and the annual mean temperature (X_2), and one month's average temperature (X_2). The average temperature (X_5), the average temperature in July (X_6) and the annual average relative humidity (X_7) have a significant correlation. Their correlation is not single, but multiple related to the geographical factors. A series of regression equations are obtained by using the SPSS 11.5 statistical software. And on the basis of the correlation analysis, two optimal models are compared.

0.366 is 1.96 residual standard deviations
8 representative areas were selected to predict the normal reference values of young men and middle-aged men in the region, and the predicted values were compared with the measured values. It was found that the predicted values of both young and middle-aged men were in good agreement with the measured values, and there was no significant difference, which explained the above regression models. The normal reference value of young male and middle-aged men in a certain area can be predicted, which provides a simple method for calculating the normal reference value of the first second forced expiratory volume of young men and middle aged and old men in a certain area from geographical factors.
By studying the relationship between the normal reference value of the forced expiratory volume in the first second of Chinese adult male and the geographical factors, the geographical distribution law of the normal reference value of the first second forced expiratory volume of the Chinese adult male was revealed, and the scientific basis was provided for the standard of formulating the normal reference value of the first second forced expiratory volume in Chinese adult male. With the rapid development of medicine, the research methods and research results of this paper will enrich the contents of medical geography.
