【摘要】: 肺順應(yīng)性和肺總量監(jiān)測(cè)都是肺功能監(jiān)測(cè)的重要指標(biāo),肺順應(yīng)性和肺總量的正常參考值在臨床上有著至關(guān)重要的作用,可以用來確切地反應(yīng)不同年齡不同性別的人的呼吸功能狀況。肺順應(yīng)性有助于深入了解肺內(nèi)氣體的傳輸和分布問題;定量研究肺順應(yīng)性的改變,對(duì)于研究慢性阻塞性肺疾病的發(fā)病機(jī)理非常重要,而且肺順應(yīng)性的測(cè)定在臨床麻醉和復(fù)蘇方面也起著重要的作用.肺總量測(cè)定是臨床診斷和疾病防治的重要依據(jù),是醫(yī)學(xué)臨床檢驗(yàn)中重要的參考指標(biāo),對(duì)判斷機(jī)體是否發(fā)生病變具有極其重要的意義。本論文的研究方法的創(chuàng)新之處在于對(duì)肺順應(yīng)性和肺總量的正常參考值與地理因素進(jìn)行了定量化的研究,并以中國(guó)整個(gè)區(qū)域?yàn)檠芯繉?duì)象,借助GIS空間分析的方法精確的繪制出正常參考值的趨勢(shì)分布圖。雖然也有少數(shù)醫(yī)學(xué)工作者的研究涉及到肺功能某些指標(biāo)的正常參考值與地理因素的關(guān)系,但也僅僅是定性描述或是簡(jiǎn)單的相關(guān)關(guān)系,逐步回歸分析等。像本文這樣對(duì)肺順應(yīng)性和肺總量的正常參考值與5項(xiàng)地理因素指標(biāo)之間運(yùn)用主成分分析方法進(jìn)行定量化和專題化的研究,從整個(gè)地理環(huán)境著手,以中國(guó)整個(gè)大區(qū)域不同年齡和性別為研究對(duì)象,國(guó)內(nèi)外還未見報(bào)道。本論文的主要研究成果是運(yùn)用主成分分析將原來共線性很大的五個(gè)地理指標(biāo)轉(zhuǎn)化為五個(gè)無共線性的變量,選擇主成分個(gè)數(shù),進(jìn)行回歸分析,得出正常參考值與地理因素的數(shù)學(xué)模型。以此模型可以方便的估算和驗(yàn)證中國(guó)某一地區(qū)不同年齡段不同性別的肺順應(yīng)性和肺總量的正常參考值,運(yùn)用GIS空間分析方法繪制出中國(guó)不同年齡不同性別肺順應(yīng)性和肺總量正常參考值的分布圖。本論文的研究方法和研究成果將豐富臨床醫(yī)學(xué),臨床檢驗(yàn)學(xué),醫(yī)學(xué)地理學(xué),環(huán)境醫(yī)學(xué)及生理學(xué)的內(nèi)容。 為了探討肺順應(yīng)性和肺總量正常參考值與地理因素之間的關(guān)系,本論文通過大量檢索文獻(xiàn),以及向有關(guān)單位求購(gòu)的方法,收集了全國(guó)各地不同年齡不同性別的肺順應(yīng)性和肺總量正常參考值的醫(yī)學(xué)指標(biāo),主要為中年人肺順應(yīng)性正常參考值、老年前期人肺順應(yīng)性正常參考值、青年男性肺總量正常參考值、中年女性肺總量正常參考值、老年前期男性肺總量正常參考值、老年前期女性肺總量正常參考值和老年人肺總量正常參考值。根據(jù)國(guó)家測(cè)繪局?jǐn)?shù)據(jù)中心提供的共享資料,國(guó)家氣象局?jǐn)?shù)據(jù)中心提供的共享資料及有關(guān)地理著作、詞典和相關(guān)文獻(xiàn),收集了五項(xiàng)地理因素指標(biāo):海拔高度(X_1)、年日照時(shí)數(shù)(X_2)、年平均氣溫(X_3)、年平均相對(duì)濕度(X_4)、年降水量(X_5)。對(duì)肺順應(yīng)性和肺總量的正常參考值與5項(xiàng)地理因素指標(biāo)之間運(yùn)用主成分分析方法進(jìn)行定量化和專題化的研究,并且建立了主成分回歸模型。其模型分別為: (1)中年人肺順應(yīng)性正常參考值與地理因素的主成分分析回歸模型:(?)=0.0757-0.00000293X_1-0.0000256X_2-0.00205X_3+0.00122X_4-0.0000150X_5±0.0191 (2)老年前期人肺順應(yīng)性正常參考值與地理因素的主成分分析回歸模型:(?)=0.0860-0.00000401X_1-0.0000341X_2-0.00231X_3+0.00147X_4-0.0000176X_5±0.0197 (3)青年男性肺總量正常參考值與地理因素的主成分分析回歸模型:(?)=6.064+0.000114X1+0.0000489X2-0.0145X3-0.00527X4-0.000265X5±0.599 (4)中年女性肺總量正常參考值與地理因素的主成分分析回歸模型:(?)=4.897+0.000305X1-0.0000702X2-0.00499X3-0.00393X4-0.0000272X5±0.630 (5)老年前期男性肺總量正常參考值與地理因素的主成分分析回歸模型:(?)=5.771+0.000278X1+0.00000935X2-0.0162X3-0.00707X4-0.000373X5±0.412 (6)老年前期女性肺總量正常參考值與地理因素的主成分分析回歸模型:(?)=4.797+0.000205X1-0.0000441X2-0.00238X3-0.00342X4-0.000247X5±0.089 (7)老年男性肺總量正常參考值與地理因素的主成分分析回歸模型:(?)=5.732+0.000132X1-0.00000331X2-0.00983X3-0.00247X4-0.0000677X5±0.493 通過上面的回歸模型,計(jì)算出各個(gè)地區(qū)不同年齡不同性別人的肺順應(yīng)性和肺總量參考值的預(yù)測(cè)值,運(yùn)用成對(duì)樣本T檢驗(yàn)的方法,檢測(cè)實(shí)測(cè)值與預(yù)測(cè)值之間的差別有無統(tǒng)計(jì)學(xué)意義。 本文選取了全國(guó)4383個(gè)市縣鄉(xiāng)鎮(zhèn)作為觀測(cè)點(diǎn),將各個(gè)觀測(cè)點(diǎn)的地理指標(biāo)帶入以上的主成分分析回歸模型中,計(jì)算出這4383個(gè)市縣鄉(xiāng)鎮(zhèn)不同性別和不同年齡人肺順應(yīng)性和肺總量正常參考值的預(yù)測(cè)值,在已矢量化好的地圖上準(zhǔn)確定位這4383個(gè)市縣鄉(xiāng)鎮(zhèn),應(yīng)用GIS軟件中的地統(tǒng)計(jì)分析模塊(Geostatistical Analyst)進(jìn)行克里格(Kriging)插值。繪制出不同性別不同年齡人肺順應(yīng)性和肺總量正常參考值分布圖。 如果我們知道了某地的地理指標(biāo),就可以用以上模型估算出該地區(qū)不同年齡不同性別的肺順應(yīng)性和肺總量正常參考值,從地理分布圖也可以得到中國(guó)任何地方的肺順應(yīng)性和肺總量正常參考值。
[Abstract]:Lung compliance and total lung volume monitoring are important indexes of lung function monitoring . The study of lung compliance and the normal reference value of total lung volume plays an important role in clinical diagnosis and recovery .
In order to study the relationship between the normal reference value of lung compliance and the normal reference value of the total lung volume and the geographical factors , this paper collects the normal reference value of the lung compliance and the normal reference value of the total lung volume in different ages of the whole country through a large number of literature searches , and the normal reference value of the total lung volume in the middle age , the average relative humidity in the middle age ( X _ 2 ) , the annual average temperature ( X _ 3 ) , the annual average relative humidity ( X _ 4 ) and the annual precipitation ( X _ 5 ) .

Based on the above regression model , the prediction value of lung compliance and lung volume reference value of different ages in different regions was calculated , and the difference between the measured value and the predicted value was detected by using the method to test the sample T .
In this paper , 4383 counties and towns in the country are selected as observation points , and the geographical indexes of each observation point are brought into the main component analysis and regression model . The prediction values of the lung compliance and the normal reference value of the total lung volume in these 4383 counties and towns are calculated , and Kriging interpolation is performed on the vectorized map . Geostatistical Analyst is applied to interpolate Kriging .
If we know a certain geographical indicator , we can use the above model to estimate the lung compliance and the normal reference value of the lung volume in different ages in the region , and the lung compliance and the normal lung volume reference value can be obtained from the geographic distribution map .
