上海市青浦區(qū)民工子弟學(xué)校新生強(qiáng)化免疫前麻疹血清流行病學(xué)橫斷面調(diào)查

【摘要】:目的掌握上海市青浦區(qū)民工子弟學(xué)校新生強(qiáng)化免疫前的麻疹免疫水平。方法采用多階段抽樣方法,抽取民工子弟學(xué)校新生606名,采用定量ELISA測定調(diào)查對象血清中的麻疹I(lǐng)gG抗體水平,同時調(diào)查就學(xué)史、麻疹相關(guān)疫苗接種史、麻疹患病史等情況。結(jié)果民工子弟學(xué)校新生強(qiáng)化免疫前麻疹I(lǐng)gG抗體幾何平均濃度(geometric mean of concentration,GMC)為998.96 mIU/mL(95%CI,912.93~1093.10),陽性率為94.88%。不同學(xué)校類別(幼托、小學(xué)、初中)對象的GMC差異無統(tǒng)計(jì)學(xué)意義(F=1.19,P=0.30),抗體陽性率差異無統(tǒng)計(jì)學(xué)意義(χ2=0.36,P=0.83);不同就學(xué)史(此前無就學(xué)史、于戶籍地就學(xué)、于上海其他學(xué)校就學(xué))對象的GMC差異無統(tǒng)計(jì)學(xué)意義(F=1.22,P=0.30),抗體陽性率差異無統(tǒng)計(jì)學(xué)意義(χ2=3.46,P=0.18);38.29%調(diào)查對象無預(yù)防接種證或已經(jīng)丟失,麻疹相關(guān)疫苗接種≥2劑次者占37.95%,接種史不詳者占48.18%,不同接種史對象的GMC差異無統(tǒng)計(jì)學(xué)意義(F=0.75,P=0.53)。結(jié)論上海市青浦區(qū)民工子弟學(xué)校新生強(qiáng)化免疫前麻疹抗體水平及抗體陽性率均處于較高水平,但不能提供明確麻疹疫苗接種史是其中一個突出問題,預(yù)防接種證的管理和入托/入園、入學(xué)接種證查驗(yàn)工作是外來民工子弟學(xué)校學(xué)生免疫規(guī)劃工作的薄弱環(huán)節(jié)。
[Abstract]:Objective to understand the measles immunization level of freshmen in Qingpu District before intensive immunization. Methods A total of 606 freshmen from the school of migrant workers were sampled by multi-stage sampling. The measles IgG antibody levels in the serum of the subjects were determined by quantitative ELISA. The history of school attendance, measles related vaccination and measles disease were investigated at the same time. Results the geometric average concentration of measles IgG antibody before intensive immunization was 998.96 mIUU / mL / 95CIQ 912.93 / 1093.10 / 10, and the positive rate was 94.88%. There was no significant difference in GMC between different school types (kindergarten, primary school, junior middle school), but there was no significant difference in antibody positive rate (蠂 ~ 2 / 0.36). There was no significant difference in GMC among the subjects enrolled in other schools in Shanghai. There was no significant difference in the positive rate of antibody between the two groups (蠂 2, 3.46, P < 0. 188.29%). 37.95 cases were inoculated with more than 2 doses of measles vaccine and 48.18 cases with unknown vaccination history. There was no significant difference in GMC between different inoculation history subjects (P < 0.53). Conclusion the level of measles antibody and the positive rate of measles antibody in the freshmen of the school of migrant workers in Qingpu District of Shanghai are all at a high level, but it is an outstanding problem to provide a clear history of measles vaccination. The administration of vaccination card and the examination of school entrance certificate are the weak links in the immunization planning of the school students of migrant workers' children.
