【摘要】：目的:回顧調查分析ICU內醫(yī)院獲得性感染(Nosocomial Infection, NI)的流行病學規(guī)律、病原體、醫(yī)院感染類型和分布,為ICU醫(yī)院感染的防治提供依據(jù)。方法:收集整理2007年1月～2009年12月入住ICU的7738份,符合醫(yī)院感染診斷標準的臨床資料進行統(tǒng)計分析。同時按標準剔除部分病例。篩選出427例病例進行回顧性分析。通過查閱病史及醫(yī)院感染登記表后統(tǒng)一填寫調查表。調查內容主要包括性別、年齡、原發(fā)病、住院天數(shù)、感染日期、易感因素、侵入性操作、抗生素使用情況、感染部位、致病菌及藥敏。對上述數(shù)據(jù)進行統(tǒng)計學分析。結果:2007～2009年ICU醫(yī)院感染感染人次率5.52%,感染例次率為15.3%,3年感染率比較具有統(tǒng)計學差異。常見感染部位依次是呼吸道、血液、術區(qū)引流、切口、中心靜脈導管、泌尿系統(tǒng)、顱內、胸膜腔等,感染構成比分別為72.1%、9.4%、4.7%、2.7%、2.1%、1.4%、1.2%、1.2%,ICU醫(yī)院獲得性感染病原菌的排名依次是鮑曼不動桿菌、金黃色葡萄球菌、銅綠假單胞菌和肺炎克雷伯菌。結論:ICU內醫(yī)院感染的患病率仍居高不下,應采取綜合措施,加強醫(yī)院感染防治,從而降低ICU感染率。
[Abstract]:Objective: to investigate and analyze the epidemiology, pathogens, types and distribution of nosocomial infection (NI) in ICU, so as to provide evidence for the prevention and treatment of nosocomial infection in ICU. Methods: 7738 patients admitted to ICU from January 2007 to December 2009 were collected and analyzed statistically. At the same time, some cases were eliminated according to the standard. 427 cases were selected for retrospective analysis. The questionnaire was completed by consulting the medical history and nosocomial infection registration form. The main contents of the investigation included sex, age, primary disease, length of stay, date of infection, susceptible factors, invasive operation, use of antibiotics, site of infection, pathogenic bacteria and drug sensitivity. The above data were analyzed statistically. Results from 2007 to 2009, the nosocomial infection rate of ICU was 5.52, and the infection rate was 15.3.The infection rate was significantly different in three years. The common infection sites were respiratory tract, blood, drainage area, incision, central venous catheter, urinary system, intracranial, pleural cavity, and so on. The infection composition ratio was 72.1and 9.4and 4.7cm, respectively. The pathogen of nosocomial infection in ICU was Acinetobacter baumannii. Staphylococcus aureus, Pseudomonas aeruginosa and Klebsiella pneumoniae. Conclusion the prevalence rate of nosocomial infection is still high in the ICU. The comprehensive measures should be taken to strengthen the prevention and treatment of nosocomial infection so as to reduce the infection rate of ICU.
