【摘要】：目的分析ICU醫(yī)院感染的危險(xiǎn)因素,并制定預(yù)防策略,以降低醫(yī)院感染率。方法對(duì)醫(yī)院ICU 2008年5月-2012年5月收治的1 030例患者臨床資料進(jìn)行回顧性分析,采用χ2檢驗(yàn)、單因素及多因素非條件logistic回歸分析,模型篩選采用Stepwise法,評(píng)價(jià)影響感染的危險(xiǎn)因素。結(jié)果共有330例患者發(fā)生醫(yī)院感染,感染率32.0%;單因素分析顯示,侵入性操作、合并基礎(chǔ)病、高齡(60歲)、長時(shí)間使用免疫抑制劑及抗菌藥物、住院時(shí)間長等均是醫(yī)院感染的危險(xiǎn)因素;而logistic回歸分析顯示,機(jī)械通氣等侵入性操作危險(xiǎn)性最大(OR5),其余依次為合并基礎(chǔ)疾病(OR=4.781)、高齡(OR=4.374)、長期使用免疫抑制劑(OR=3.647)、長期使用抗菌藥物(OR=3.547)、住院時(shí)間長(OR=2.247)。結(jié)論 ICU醫(yī)院感染的危險(xiǎn)因素較多,臨床上應(yīng)采取針對(duì)性的預(yù)防對(duì)策,從而使感染率得到有效控制。
[Abstract]:Objective to analyze the risk factors of ICU nosocomial infection and to make preventive strategies to reduce nosocomial infection rate.Methods the clinical data of 1 030 patients with ICU from May 2008 to May 2012 were retrospectively analyzed. The risk factors of infection were evaluated by 蠂 2 test, univariate and multivariate logistic regression analysis and Stepwise method.Results A total of 330 patients developed nosocomial infection and the infection rate was 32.0.The univariate analysis showed that invasive operation, complicated with basic diseases, aged 60 years old, and prolonged use of immunosuppressive agents and antibiotics,The length of stay in hospital was the risk factor of nosocomial infection, and logistic regression analysis showed that,The risk of invasive operation, such as mechanical ventilation, was the greatest, and the others were OR4.781, 4.374, 3.647, 3.547, and 2.247, respectively.Conclusion there are many risk factors for ICU nosocomial infection.
