時—空掃描統(tǒng)計量在傳染病早期預(yù)警中的應(yīng)用研究

【摘要】: 目的傳染病聚集性的早期發(fā)現(xiàn)是有效控制傳染病爆發(fā)與流行、降低傳染病發(fā)病率和死亡率的重要措施。但目前我國用于傳染病聚集性探測的方法僅限于單純的時間聚集性探測方法,不能充分利用數(shù)據(jù)信息,影響了聚集性探測的及時性和有效性。本研究旨在探討時空掃描統(tǒng)計量這一先進的統(tǒng)計技術(shù)在傳染病聚集性探測以及早期預(yù)警中的應(yīng)用價值。 方法模擬實時監(jiān)測系統(tǒng),采用時空掃描統(tǒng)計量建立模型,對我國2005年麻疹病例數(shù)據(jù)進行逐日前瞻性分析,并分別以省、區(qū)縣和鄉(xiāng)鎮(zhèn)街道作為基本地理單位建立預(yù)警模型,在此基礎(chǔ)上,探討了探測結(jié)果三維可視化實現(xiàn)的途徑和方法。 結(jié)果全國省級分析結(jié)果顯示:前瞻性時-空掃描統(tǒng)計量可提供確切的預(yù)警信息,對麻疹聚集性進行時間和空間定位;上海市、江蘇省和浙江省的區(qū)縣級分析結(jié)果顯示:前瞻性時-空掃描統(tǒng)計量能及時探測到開始于某省局部縣區(qū)和三省交界區(qū)縣的麻疹聚集性。成都市鄉(xiāng)鎮(zhèn)街道級分析結(jié)果顯示:在缺乏人口數(shù)據(jù)的情況下,前瞻性時-空重排掃描統(tǒng)計量仍能有效進行預(yù)警,且較前瞻性時間掃描統(tǒng)計量能更早探測到麻疹聚集性。 結(jié)論時空掃描統(tǒng)計量能夠充分挖掘和利用傳染病監(jiān)測數(shù)據(jù)中的時間和空間信息,提高聚集性探測的及時性和有效性,能夠在各級行政區(qū)劃早期探測到傳染病在時空兩個維度的聚集性。利用前瞻性時-空掃描統(tǒng)計量模型結(jié)合地理信息軟件建立的早期預(yù)警系統(tǒng),能夠有效地做出早期預(yù)警。時空掃描統(tǒng)計量在實際工作中具有潛在的重要應(yīng)用價值。
[Abstract]:Objective the early detection of infectious disease aggregation is to effectively control the outbreak and prevalence of infectious diseases. The important measures to reduce the morbidity and mortality of infectious diseases. However, at present, the methods used to detect the aggregation of infectious diseases in our country are limited to the simple method of detecting the aggregation of infectious diseases, and the data information is not fully utilized. The purpose of this study is to explore the application value of spatio-temporal scanning statistics, an advanced statistical technique, in the detection and early warning of infectious diseases. Methods the real-time surveillance system was simulated, and the model was established by space-time scanning statistics. The measles case data in 2005 in China were analyzed day by day, and the early warning model was established with provincial, district, county and township streets as the basic geographical units respectively. On this basis, the approaches and methods to realize 3D visualization of detection results are discussed. Results the results of provincial level analysis showed that the prospective time-space scanning statistics could provide accurate early warning information and locate the aggregation of measles in time and space. The results of district and county level analysis in Jiangsu province and Zhejiang province show that the prospective time-space scanning statistics can detect the measles aggregation in some counties and the borderline counties of three provinces in time. Analysis of Township and Township Street level in Chengdu. Shows: in the absence of population data, Prospective time-space rearrangement scan statistics can still be used for early warning, and measles aggregation can be detected earlier than prospective time-scanning statistics. Conclusion Spatio-temporal scanning statistics can fully mine and utilize the temporal and spatial information of infectious disease surveillance data, and improve the timeliness and effectiveness of aggregation detection. Early detection of the aggregation of infectious diseases in two dimensions of time and space in administrative divisions at all levels. An early warning system based on prospective time-space scanning statistics model and geographic information software, Spatio-temporal scanning statistics have potential important application value in practical work.
