高頻接觸物體表面消毒質(zhì)量管理在多藥耐藥菌控制中的作用研究

【摘要】:目的探討ICU高頻接觸物體表面消毒質(zhì)量管理在多藥耐藥菌控制中的作用,以提高醫(yī)療質(zhì)量。方法選擇2013年5月-2014年12月ICU 1 297例住院患者的多藥耐藥菌檢出結(jié)果為研究對(duì)象,將2013年5月-2014年2月住院患者594例設(shè)為干預(yù)前組,2014年3-12月住院患者703例設(shè)為干預(yù)后組,由責(zé)任護(hù)士負(fù)責(zé)組內(nèi)高頻接觸物體表面的消毒,并規(guī)范消毒方法和消毒質(zhì)量的督查,比較兩組的消毒質(zhì)量和多藥耐藥菌檢出結(jié)果。結(jié)果高頻接觸物體表面消毒質(zhì)量在干預(yù)前后合格率分別為80.3%和94.3%,差異有統(tǒng)計(jì)學(xué)意義(P0.05);干預(yù)前后目標(biāo)性病原菌監(jiān)測(cè)多藥耐藥菌檢出率分別為77.9%和37.5%,差異有統(tǒng)計(jì)學(xué)意義(P0.05);ICU患者目標(biāo)性病原菌監(jiān)測(cè)多藥耐藥菌檢出率在干預(yù)前后分別為45.2%和26.3%,其中耐甲氧西林金黃色葡萄球菌、耐碳青霉烯類肺炎克雷伯菌和泛耐藥鮑氏不動(dòng)桿菌檢出率顯著下降,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論重視并規(guī)范高頻接觸物體表面消毒質(zhì)量管理,能有效控制多藥耐藥菌在醫(yī)院內(nèi)的傳播。
[Abstract]:Objective to investigate the effect of ICU high frequency contact surface disinfection quality management on the control of multidrug resistant bacteria (MDR) in order to improve the medical quality. Methods the results of multidrug resistance (MDR) detection in ICU 1 297 inpatients from May 2013 to December 2014 were selected as the research objects. From May 2013 to February 2014, 594 inpatients were divided into pre-intervention group, and 703 inpatients from 2014 to December were selected as post-intervention group. The responsible nurses were responsible for the disinfection of the surface of high frequency contact objects in the group and the supervision of disinfection methods and disinfection quality. Results the qualified rates of disinfection quality of high frequency contact objects before and after intervention were 80.3% and 94.3 respectively, the difference was statistically significant (P 0.05). The detection rates of drug-resistant bacteria were 77.9% and 37.5, respectively. There was significant difference in the detection rate of multidrug resistant bacteria in patients with P0.05ICU. The detection rates of multidrug resistant bacteria were 45.2% and 26.3 before and after intervention respectively, among which methicillin-resistant Staphylococcus aureus was found to be resistant to methicillin-resistant Staphylococcus aureus. The detection rate of Klebsiella pneumoniae and Acinetobacter baumannii decreased significantly, and the difference was statistically significant (P 0.05). Conclusion the quality management of surface disinfection of high frequency contact objects should be emphasized and standardized. It can effectively control the spread of multidrug resistant bacteria in hospitals.
