【摘要】：我國是全球22個(gè)結(jié)核病高負(fù)擔(dān)國家之一。2000年調(diào)查顯示，我國有結(jié)核病病人451萬，，結(jié)核病病人總數(shù)居世界第二位。與一般人群相比較，監(jiān)獄系統(tǒng)結(jié)核病疫情更為嚴(yán)重。長期以來，我國監(jiān)獄系統(tǒng)結(jié)核病一直是危害服刑者身體健康的主要疾病。據(jù)2002年9月至2003年3月進(jìn)行的全國監(jiān)獄系統(tǒng)結(jié)核病防治工作調(diào)研報(bào)告顯示，我國監(jiān)獄系統(tǒng)有活動(dòng)性肺結(jié)核19171例，患病率高達(dá)1251/10萬，為全國平均水平的3.4倍。 近年來，國際上越來越多的研究者開始對(duì)監(jiān)獄系統(tǒng)結(jié)核病疫情以及衛(wèi)生服務(wù)利用情況進(jìn)行研究。不同國家監(jiān)獄服刑人員衛(wèi)生服務(wù)利用情況特點(diǎn)不一。就我國而言，目前未見對(duì)監(jiān)獄系統(tǒng)結(jié)核病服刑人員衛(wèi)生服務(wù)利用及影響因素方面的研究。為此，我們?cè)谑澜缧l(wèi)生組織的資助下，對(duì)山東、遼寧兩省部分監(jiān)獄在押的255名結(jié)核病病犯進(jìn)行了衛(wèi)生服務(wù)利用及影響因素方面的調(diào)查研究，旨在了解山東省與遼寧省監(jiān)獄結(jié)核病防治服務(wù)現(xiàn)狀，評(píng)價(jià)結(jié)核病防治服務(wù)的影響因素，探索符合監(jiān)獄系統(tǒng)實(shí)際的結(jié)核病防治策略，為有關(guān)部門制定政策提供合理建議。 本研究采用個(gè)案研究的方法，在山東省選擇三所監(jiān)獄(其中，兩所普通監(jiān)獄，一所醫(yī)療監(jiān)獄，即專門為病犯提供醫(yī)療服務(wù)的監(jiān)獄)，在遼寧省選擇一所監(jiān)獄進(jìn)行調(diào)查和分析。根據(jù)監(jiān)獄結(jié)核病患病情況以及監(jiān)獄的性質(zhì)，在山東選擇了患病率相對(duì)高的濰坊監(jiān)獄、山東省監(jiān)獄與新康監(jiān)獄(醫(yī)療監(jiān)獄)進(jìn)行調(diào)查；在遼寧選擇了鐵嶺監(jiān)獄(醫(yī)療監(jiān)獄)進(jìn)行調(diào)查。資料主要來源于文獻(xiàn)檢索、機(jī)構(gòu)調(diào)查、肺結(jié)核病犯問卷調(diào)查和關(guān)鍵人物訪談。
[Abstract]:China is one of the 22 countries with high TB burden in the world. In 2000, the survey showed that there are 4.51 million TB patients in China, and the total number of TB patients ranks second in the world. Compared with the general population, the tuberculosis epidemic in prison system is more serious for a long time. Tuberculosis in the prison system in China has always been a major disease endangering the health of prisoners. According to the report of the National investigation on Tuberculosis Control in the Prison system from September 2002 to March 2003, there are 19171 cases of active tuberculosis in the prison system of our country. The prevalence rate reached 1251/10, 000, 3.4 times the national average. In recent years, more and more international researchers have begun to study the situation of tuberculosis in the prison system and the utilization of health services. The characteristics of the utilization of health services for prison inmates in different countries are different. As far as our country is concerned, At present, no research has been done on the utilization of health services and the factors affecting the utilization of health services for tuberculosis inmates in the prison system. For this reason, we have, with the financial assistance of the World Health Organization, made a study on the situation in Shandong, In order to understand the present situation of tuberculosis control services in prisons of Shandong Province and Liaoning Province, 255 TB prisoners in two provinces of Liaoning Province were investigated in terms of the utilization of health services and the influencing factors. To evaluate the influencing factors of TB prevention and control service, to explore the tuberculosis prevention and control strategy in accordance with the actual situation of prison system, and to provide reasonable suggestions for relevant departments to formulate policies. This study adopts the case study method to select three prisons (two ordinary prisons, one medical prison) in Shandong Province. That is, a prison that specializes in providing medical services to sick prisoners, selected a prison in Liaoning Province for investigation and analysis. According to the prevalence of tuberculosis in prisons and the nature of the prisons, Weifang Prison, which has a relatively high prevalence rate, was selected in Shandong Province. Shandong prison and Xinkang prison (medical prison) were investigated; Tieling prison (medical prison) was selected for investigation in Liaoning Province.
