DWI及動(dòng)態(tài)增強(qiáng)MRI鑒別診斷舌良惡性病變

【摘要】:目的評(píng)價(jià)DWI及動(dòng)態(tài)增強(qiáng)MRI鑒別診斷舌良惡性病變的價(jià)值。方法對(duì)75例經(jīng)病理確診的舌部病變患者行DWI和動(dòng)態(tài)增強(qiáng)MRI。測(cè)量并比較舌良惡性病變病灶A(yù)DC值,繪制ROC曲線分析ADC值診斷舌惡性腫瘤效能;比較舌良惡性病變時(shí)間-信號(hào)強(qiáng)度曲線(TIC)分型差異。結(jié)果舌惡性病變的ADC值[(1.12±0.21)×10-3 mm2/s]明顯低于良性病變[(1.84±0.47)×10-3 mm2/s,P0.001);以ADC值1.30×10-3 mm2/s為閾值,診斷舌惡性病變的敏感度、特異度和準(zhǔn)確率分別為90.70%、93.75%和92.00%,ROC曲線下面積為0.957。良性病變(除外8例囊腫)中19例(19/24,79.17%)TIC為Ⅲ型,5例(5/24,20.83%)為Ⅱ型;惡性病變中39例(39/43,90.70%)TIC為Ⅰ型,4例(4/43,9.30%)為Ⅱ型。結(jié)論舌良惡性病變DWI和動(dòng)態(tài)增強(qiáng)MRI具有不同特征,ADC值和TIC曲線有助于鑒別診斷兩者。
[Abstract]:Objective to evaluate the value of DWI and dynamic enhanced MRI in differential diagnosis of benign and malignant lesions of tongue. Methods DWI and dynamic enhanced MRI. were performed in 75 patients with pathological diagnosis of tongue lesions. The ADC values of benign and malignant lesions of tongue were measured and compared, and the ROC curve was drawn to analyze the diagnostic efficacy of ADC values in the diagnosis of tongue malignant tumors, and the time-signal intensity curve of benign and malignant lesions of tongue was compared with the difference of (TIC) classification. Results the ADC value in malignant lesions of tongue [(1.12 鹵0.21) 脳 10 ~ (3) mm2/s] was significantly lower than that in benign lesions [(1.84 鹵0.47) 脳 10 ~ (3) mm2/s,P0.001]. The sensitivity, specificity and accuracy of the diagnosis of tongue malignant lesions were 90.70%, 93.75% and 92.00%, respectively, and the area under the curve was 0.957 using the ADC value of 1.30 脳 10 ~ 3 mm2/s as the threshold value, and the sensitivity, specificity and accuracy were 90.70%, 93.75% and 92.0% respectively.
