【摘要】：目的探討口腔頜面部多間隙感染的病原菌分布及臨床治療方法,為口腔頜面部多間隙感染的治療提供依據(jù)。方法調(diào)查2011年10月-2013年5月100例口腔頜面部多間隙感染患者臨床資料,依據(jù)隨機(jī)數(shù)字表法分為觀察組和對(duì)照組,每組各50例;對(duì)照組患者給予常規(guī)對(duì)癥治療,觀察組患者在常規(guī)對(duì)癥治療基礎(chǔ)上行膿腫切開引流聯(lián)合高壓氧治療,采用SPSS21.0進(jìn)行統(tǒng)計(jì)分析。結(jié)果口腔頜面部多間隙感染最常見的病原菌為金黃色葡萄球菌和星座鏈球菌星座亞種;觀察組患者中顯效29例、有效19例,治療總有效率為96.0%;對(duì)照組患者中顯效9例、有效31例,治療總有效率為80.0%;觀察組患者治療的總有效率明顯比對(duì)照組高(P0.05);觀察組患者治療后白細(xì)胞計(jì)數(shù)明顯比對(duì)照組低(P0.05);觀察組患者發(fā)生敗血癥1例,術(shù)后并發(fā)癥發(fā)生率為2.0%;對(duì)照組患者發(fā)生敗血癥3例、縱隔炎及心力衰竭各2例,術(shù)后并發(fā)癥發(fā)生率為14.0%,觀察組患者術(shù)后并發(fā)癥發(fā)生率明顯低于對(duì)照組(P0.05)。結(jié)論口腔頜面部多間隙感染的病原菌主要是金黃色葡萄球菌和星座鏈球菌星座亞種,常規(guī)對(duì)癥治療基礎(chǔ)上的膿腫切開引流聯(lián)合高壓氧治療具有良好的臨床療效。
[Abstract]:Objective to investigate the distribution of pathogenic bacteria in oral and maxillofacial multi-space infection and its clinical treatment, so as to provide evidence for the treatment of oral and maxillofacial multi-space infection. Methods from October 2011 to May 2013, 100 patients with oral and maxillofacial multi-space infection were divided into observation group (n = 50) and control group (n = 50). The patients in the control group were treated with conventional symptomatic therapy. The patients in the observation group were treated with abscess incision and drainage combined with hyperbaric oxygen therapy on the basis of routine symptomatic treatment. SPSS21.0 was used for statistical analysis. Results Staphylococci aureus and Streptococcus astrococcus subspecies were the most common pathogens in oral and maxillofacial multi-space infection, in the observation group, 29 cases were effective and 19 cases were effective, the total effective rate was 96.0%. The total effective rate was 80.0% in the control group, and the total effective rate in the observation group was significantly higher than that in the control group (P0.05), and the total effective rate in the observation group was significantly higher than that in the control group (P0.05). After treatment, the leukocyte count in the observation group was significantly lower than that in the control group (P0.05), one patient in the observation group had septicemia, and the incidence of postoperative complications was 2.0%. There were 3 cases of septicemia, 2 cases of mediastinal inflammation and 2 cases of heart failure in the control group. The incidence of postoperative complications was 14.0%. The incidence of postoperative complications in the observation group was significantly lower than that in the control group (P0.05). Conclusion Staphylococcus aureus and Streptococcus astrococci are the main pathogens of multiple space infections in oral and maxillofacial regions. The conventional treatment of abscess incision and drainage on the basis of symptomatic treatment combined with hyperbaric oxygen therapy has a good clinical effect.
