平均生長型青少年不同矢狀骨面型切牙與磨牙傾斜度研究

【摘要】:目的:通過對平均生長型青少年三種矢狀骨面型治療前頭顱定位側(cè)位片進(jìn)行測量,比較三種不同矢狀骨面型切牙唇舌向和第一恒磨牙近遠(yuǎn)中傾斜度代償特點及差異,為臨床矯治設(shè)計、支抗預(yù)備及正畸術(shù)后長期穩(wěn)定提供參考依據(jù)。方法:依據(jù)納入標(biāo)準(zhǔn)與面部生長型分型標(biāo)準(zhǔn)從正畸患者中選取平均生長型青少年120例,按ANB角行矢狀向分型:Ⅰ類骨面型(ANB角0°-5°),Ⅱ類骨面型(ANB角≥5°),Ⅲ類骨面型(ANB角≤0°)三組每組各40例,使用13個角度作為切牙及第一恒磨牙傾斜度的測量指標(biāo),三組進(jìn)行單因素方差分析及兩兩比較用LSD-t檢驗。結(jié)果:1.不同矢狀骨面型切牙傾斜度測量結(jié)果:三組不同的矢狀骨面型中,∠U1-SN、∠U1-FH、∠U1-PP的測量值差異均無統(tǒng)計學(xué)意義;∠L1-MP的測量值三組間的差異有統(tǒng)計學(xué)意義,骨性Ⅲ類∠L1-MP的數(shù)值小于Ⅰ類、Ⅱ類,骨性Ⅰ類、Ⅱ類之間無區(qū)別;∠U1-L1的測量值三組間差異有統(tǒng)計學(xué)意義,骨性Ⅲ類∠U1-L1的數(shù)值大于Ⅰ類、Ⅱ類,骨性Ⅰ類、Ⅱ類之間無區(qū)別。2.不同矢狀骨面型第一恒磨牙傾斜度測量結(jié)果:∠U6-SN、∠U6-FH、∠U6-PP、∠L6-SN均是骨性Ⅲ類組大于骨性Ⅰ類、Ⅱ類組,∠L6-MP骨性Ⅲ類組小于Ⅰ類、Ⅱ類組,差異均有統(tǒng)計學(xué)意義;∠U6-L6的測量值三組間差異無統(tǒng)計學(xué)意義。下頜切牙與磨牙傾斜度的測量值均是骨性Ⅲ類組的小于Ⅰ類、Ⅱ類,提示骨性Ⅲ類下切牙、下頜第一恒磨牙代償性傾斜一致;骨性Ⅲ類下切牙較Ⅰ類代償變化角度為9.24°,下頜第一恒磨牙較Ⅰ類代償變化角度為4.11°,提示骨性Ⅲ類下切牙的代償性比下磨牙的明顯。結(jié)論:1.隨著ANB角的減小,平均生長型青少年下頜切牙舌向傾斜,上頜第一恒磨牙近中傾斜,下頜第一恒磨牙遠(yuǎn)中傾斜,上頜切牙唇舌向傾斜度在三種骨面型中無差別。2.平均生長型青少年骨性Ⅲ類上頜第一恒磨牙近中傾斜代償,下頜第一恒磨牙遠(yuǎn)中傾斜代償,下切牙舌向傾斜代償;下切牙與磨牙代償傾斜趨勢一致,下頜切牙代償較磨牙明顯。3.骨性Ⅱ類與Ⅰ類比較,上切牙趨于舌向傾斜,上頜第一恒磨牙趨于遠(yuǎn)中傾斜,下切牙趨于唇向傾斜,下頜第一恒磨牙趨于近中傾斜。
