牙周基礎(chǔ)治療對肥胖大鼠實驗性牙周炎及代謝水平的影響

【摘要】:目的探討牙周干預(yù)治療肥胖合并牙周炎的大鼠,對大鼠牙周炎的控制、胰島素抵抗及脂代謝水平的影響。方法將SD大鼠隨機分為正常組(C組)、肥胖組(O組)、牙周炎合并肥胖組(P組)和牙周治療組(T組)。P組和T組大鼠行上頜第2磨牙牙周結(jié)扎,T組大鼠做基礎(chǔ)牙周治療,所有大鼠24周齡處死。眼眶靜脈取血檢測血脂四項,胰島素和血糖,計算胰島素抵抗(HOMA穩(wěn)態(tài)模型胰島素抵抗值,HOMA-IR)。取大鼠頜骨及各臟器行病理觀察;實時定量PCR法檢測肝臟中胰島素受體底物-1(IRS-1),胰島素受體底物-2(IRS-2)的表達。結(jié)果 P組與單純肥胖O組比較:HOMA-IR、LDL-C值均出現(xiàn)升高;而IRS-1、IRS-2表達及HDL-C值則低于肥胖組。T組與P組比較,大鼠的胰島素抵抗HOMA-IR值下降(P0.05);IRS-1及IRS-2的mRNA表達均有增高(P0.05),TG值變化不明顯(P0.05),HDL-C升高,LDL-C及TC值出現(xiàn)下降(P0.05)。病理學(xué)觀察:T組大鼠頜骨標(biāo)本切片中的炎細胞浸潤情況出現(xiàn)好轉(zhuǎn)。P組大鼠頜骨破壞最嚴重。肝臟切片比較,P組大鼠的肝細胞中肝脂變最為廣泛,炎癥浸潤最嚴重。結(jié)論牙周的炎癥可下調(diào)IRS-1,IRS-2的表達,加重肥胖大鼠的胰島素抵抗和血脂代謝紊亂。牙周治療對于改善肥胖大鼠胰島素抵抗的情況,減輕血脂代謝紊亂有一定的影響。
[Abstract]:Objective to investigate the effects of periodontal intervention on periodontitis control, insulin resistance and lipid metabolism in obese rats with periodontitis. Methods SD rats were randomly divided into normal group (C group), obese group (O group), periodontitis combined with obesity group (P group) and periodontal treatment group (T group). P and T group rats) were treated with maxillary second molar periodontal ligation. Rats in group T were treated with basic periodontal therapy and all rats were killed at 24 weeks old. Serum lipids, insulin and blood glucose were measured by orbital vein blood collection. Insulin resistance (HOMA steady-state model insulin resistance, HOMA-IR) was calculated. The expression of insulin receptor substrate-1 (IRS-1) and insulin receptor substrate-2 (IRS-2) in the liver were detected by real-time quantitative PCR. Results compared with simple obesity group O, the HOMA-IR,LDL-C value of P group was higher than that of simple obesity O group. The expression of IRS-1,IRS-2 and the value of HDL-C were lower in group T than those in group P. the HOMA-IR value of insulin resistance in group T was lower than that in group P (P0.05). The mRNA expression of IRS-1 and IRS-2 were increased (P0.05), TG value was not significantly changed (P0.05), HDL-C increased, LDL-C and TC values decreased (P0.05). Pathological observation: the infiltration of inflammatory cells in group T was improved, and the destruction of jaw was the most serious in group P. In group P, liver lipid was the most extensive and inflammatory infiltration was the most serious. Conclusion periodontal inflammation can down-regulate the expression of IRS-1,IRS-2 and aggravate insulin resistance and dyslipidemia in obese rats. Periodontal therapy has a certain effect on improving insulin resistance and reducing lipid metabolism disorder in obese rats.
