成人骨性Ⅱ類正畸治療后面部軟硬組織的變化分析

【摘要】:目的:通過對成人骨性II類患者進行正畸矯治,,對比矯治前后評價軟組織側(cè)貌美學(xué)指標(biāo)的變化及相關(guān)硬組織測量指標(biāo)的變化情況,對骨性II類的成年患者正畸治療后面部軟硬組織的變化進行探討。 方法:選取沈陽市口腔醫(yī)院2007年4月收治的成人骨性II類患者一例,女性,23歲,上頜右側(cè)第一磨牙和左側(cè)第二前磨牙因齲拔除,下牙弓擁擠53,前牙深笐M籃螴°、深覆蓋II°,下頜中線右偏23,尖褲硣嵭关蠐(dān)也嗄パ萊試噸屑舛約夤叵怠;頰呃湊鎦魎哐萊薟黃、嘴凸,想讣{潑嫘,因此通过对其进行临床空樬、宛h癤線檢查,結(jié)合頭顱側(cè)位片所測量數(shù)據(jù)和患者想改善面型的強烈愿望,制定拔除35和45的矯治方案,排齊整平牙列,利用種植體支抗,關(guān)閉拔牙間隙,最大程度回收前牙,調(diào)整磨牙咬合關(guān)系。矯治前后拍攝X線頭顱側(cè)位片。選取E線,33項軟、硬組織測量指標(biāo)及側(cè)貌美學(xué)指標(biāo)判斷方程,對比矯治前后軟、硬組織的變化情況。 結(jié)果:矯治療程28個月。治療結(jié)束后患者前牙覆牙合覆蓋關(guān)系正常,牙齒排列整齊,牙齒中線與面中線一致,居中對齊,雙側(cè)尖牙中性關(guān)系;雙側(cè)顳下頜關(guān)節(jié)檢查未見異常,開口度及開口型正常,全口牙齒達尖窩相對咬合關(guān)系、無干擾。通過正畸治療許多美學(xué)指標(biāo)及其相關(guān)的硬組織測量指標(biāo)發(fā)生了變化:(1)U1-SN、U1-NA、L1-MP、L1-NB均減。2)MP-FH、MP-SN、Y軸角增加(3)UL-EP、LL-EP、鼻唇溝深度、頦唇溝深度、上唇位置、下唇位置、下唇凹點厚度均減。4)鼻唇角、上唇傾角、下唇傾角、Z角、頦唇溝傾角、面凸角、頦唇角、軟組織面角均增大。正畸治療后軟組織側(cè)貌由凸趨于直,側(cè)貌改善至美觀,患者滿意。 結(jié)論:(1)正畸治療可以使成人骨性II類患者側(cè)貌明顯改善。(2)成人患者正畸治療后側(cè)貌改善主要與上下前牙的位置相關(guān);上下頜骨矢狀向位置未發(fā)生變化。(3)成人骨性II類患者應(yīng)用種植體支抗可以實現(xiàn)最大程度的內(nèi)收前牙,改善面型。
[Abstract]:Objective: to evaluate the changes of soft tissue side appearance aesthetic indexes and related hard tissue indexes before and after orthodontic treatment in adult patients with bone II. To investigate the changes of soft and hard facial tissues after orthodontic treatment in adult patients with bone II. Methods: a female, aged 23, with right first molar of maxilla and left second premolar, was selected from adult bone II group admitted in Shenyang Stomatology Hospital in April, 2007. Results: the treatment course was 28 months. Through orthodontic treatment, many aesthetic indexes and their related hard tissue measurements have been changed: (1) U1-SNU1-NAA L1-MPL1-NB decreased (2) MP-FH,MP-SN,Y axis angle increased (3) UL-EP, The depth of LL-EP, nasolabial sulcus, the depth of genial-labial sulcus, the position of upper lip, the thickness of lower lip concave point were all decreased (4) nasal lip angle, upper lip inclination angle, lower lip inclination angle, Z angle, geniolabial groove inclination angle, face convex angle, chin lip angle. The soft tissue facial angle increased. After orthodontic treatment, the soft tissue profile changed from convex to straight and improved to beautiful. Conclusion: (1) Orthodontic treatment can significantly improve the lateral appearance of adult patients with bone II. (2) the improvement of posterior side appearance of adult patients with orthodontic treatment is mainly related to the position of upper and lower anterior teeth. The sagittal position of the upper and lower bones did not change. (3) Implant Anchorage could achieve maximum adductive anterior teeth and improve facial shape in adult patients with II.
