種植體支抗不同加力方式下關(guān)閉拔牙間隙的比較

【摘要】:研究目的:在植入微螺釘種植支抗的前提下,對(duì)鎳鈦拉簧和鏈狀橡皮圈關(guān)閉拔牙間隙的療效進(jìn)行比較。 研究方法:篩選2011年12月—2013年8月就診于山西醫(yī)科大學(xué)第一醫(yī)院口腔正畸科被診斷為上頜前突的正畸患者29例,選取符合條件者24例,將24例上頜前突正畸患者隨機(jī)分為兩組,鎳鈦拉簧組12例,鏈狀橡皮圈組12例,正畸矯治均需拔除雙側(cè)上、下頜第一前磨牙。24例患者在常規(guī)排齊整平牙列后,于雙側(cè)上、下頜第二前磨牙與第一磨牙之間,各植入1枚微螺釘種植體并即刻加載,分別應(yīng)用鎳鈦拉簧或鏈狀橡皮圈配合MBT直絲弓矯治技術(shù),采用一步滑動(dòng)法整體內(nèi)收前牙,直至完全關(guān)閉拔牙間隙。兩組患者關(guān)閉拔牙間隙前、后均拍攝頭顱定位側(cè)位片并制取牙模,記錄兩組患者關(guān)閉上、下頜拔牙間隙所用的時(shí)間,用SPSS13.0統(tǒng)計(jì)軟件包對(duì)兩組病例數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析,比較兩組患者關(guān)閉拔牙間隙前、后上頜中切牙的位置變化及關(guān)閉拔牙間隙所需時(shí)間的差異。 研究結(jié)果: 微螺釘支抗在垂直向、矢狀向的位置變化均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);關(guān)閉拔牙間隙后,在垂直方向上,鎳鈦拉簧組壓低上頜中切牙(4.13+2.36)mm,鏈狀橡皮圈組壓低上頜中切牙(1.63+1.99)mm,,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);在矢狀向上,鎳鈦拉簧組上頜中切牙內(nèi)收(8.00+3.45)mm,鏈狀橡皮圈組上頜中切牙內(nèi)收(7.71+1.98)mm,兩組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);③鎳鈦拉簧組關(guān)閉上頜拔牙間隙所用時(shí)間為(6.10+0.35)月,鏈狀橡皮圈組關(guān)閉上頜拔牙間隙所用時(shí)間為(8.20+0.28)月,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);④對(duì)于鎳鈦拉簧組和鏈狀橡皮圈組患者關(guān)閉下頜拔牙間隙所需的時(shí)間分別進(jìn)行組間比較,結(jié)果發(fā)現(xiàn),高角組與其余兩組相比,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。 研究結(jié)論: 鎳鈦拉簧組較鏈狀橡皮圈組,在相同時(shí)間內(nèi)、同一力值作用下,可以更大程度地壓低上頜前牙,明顯改善前牙露齦笑及深覆牙合;鎳鈦拉簧和鏈狀橡皮圈,均可以最大限度地內(nèi)收上頜前牙,明顯改善前牙深覆蓋,差異無(wú)顯著性; MBT直絲弓矯治技術(shù)一步滑動(dòng)法關(guān)閉上頜拔牙間隙,使用鎳鈦拉簧較鏈狀橡皮圈,關(guān)閉拔牙間隙所需的時(shí)間明顯縮短,對(duì)于上頜前突的正畸患者臨床效果好;③無(wú)論是鎳鈦拉簧組還是鏈狀橡皮圈組,在關(guān)閉下頜拔牙間隙時(shí),高角病人較均角、低角病人所需時(shí)間明顯縮短,差異有顯著性。
[Abstract]:Objective: to compare the efficacy of nickel titanium spring and chain rubber band in closing the extraction space under the condition of implant Anchorage with microscrews. Methods: from December 2011 to August 2013, 29 orthodontic patients who were diagnosed as maxillary protrusion in Department of Orthodontics, first Hospital of Shanxi Medical University, were selected. Twenty-four patients with maxillary protrusion were randomly divided into two groups: Ni-Ti tension group (n = 12) and chain rubber band group (n = 12). Between the second mandibular premolars and the first molars, one microscrew implant was implanted and immediately loaded. The single step sliding method was used to treat the anterior teeth with nickel titanium spring or chain rubber band combined with MBT straight wire appliance, respectively. Until the extraction space is completely closed. Before closing the extraction space, the patients in both groups were photographed the lateral position of the head and made the tooth model. The time of the two groups were recorded. The data of the two groups were statistically analyzed by SPSS13.0 software package. The changes of the position of the maxillary central incisor and the time needed to close the extraction space were compared between the two groups. Results: there was no significant change in the position of microscrew Anchorage in vertical and sagittal direction (P0.05). After closing the extraction space, in the vertical direction, the Niti pull spring group depressed the maxillary central incisor (4.132.36) mm, chain rubber band group depressed the maxillary central incisor (1.63 1.99) mm,. The difference between the two groups was statistically significant (P0.05). In sagittal direction, the adduction of maxillary central incisor in Niti Reed group (8.003.45) mm, chain rubber band group (7.71.98) mm, had no significant difference between the two groups (P0.05). (3) the time to close the maxillary extraction space was (6.100.35) months in the Ni-Ti tension spring group and (8.20 0.28) months in the chain rubber band group. The difference between the two groups was statistically significant (P0.05). 4 the time required to close the mandibular extraction space was compared between the nickel titanium tension spring group and the chain rubber ring group. The results showed that there were significant differences between the high angle group and the other two groups (P0.05). Conclusion: compared with the chain rubber band group, the Ni-Ti tension spring group can lower the maxillary anterior teeth more significantly and improve the gingival smile and deep overbite at the same time and under the same force value. Both Nickel-titanium spring and chain rubber band could retract the maxillary anterior teeth to the maximum extent and significantly improve the depth of the anterior teeth, but there was no significant difference. The one-step sliding method of MBT straight-wire appliance was used to close the maxillary extraction space. The time needed to close the extraction space was shortened obviously by using the Ni-Ti spring than the chain rubber band. The clinical effect was good for the patients with maxillary protruding orthodontic treatment. (3) when the mandibular extraction space was closed, the patients with high angle and the patients with low angle had significantly shorter time when closing the mandibular extraction space, whether in the nickel-titanium spring group or in the chain rubber band group, and the time required for the patients with lower angle was significantly shorter than that in the patients with lower angle.
