降鈣素對(duì)大鼠上頜擴(kuò)弓初期影響的研究

【摘要】:[目的]正畸治療中,上頜擴(kuò)弓可以有效的糾正患者牙弓水平方向發(fā)育不足引起的錯(cuò)(?)畸形,然而多方面因素的影響使得上頜擴(kuò)弓的效果不穩(wěn)定,存在一定的復(fù)發(fā)率。降鈣素是甲狀腺濾泡旁細(xì)胞分泌產(chǎn)生的一種多肽類激素,可以調(diào)節(jié)全身鈣離子流,促進(jìn)鈣鹽沉積以及骨質(zhì)改建。然而降鈣素對(duì)上頜擴(kuò)弓的影響尚無(wú)相關(guān)研究。本實(shí)驗(yàn)擬通過(guò)建立大鼠上頜擴(kuò)弓模型,探討降鈣素對(duì)大鼠上頜擴(kuò)弓的影響,以便為將來(lái)能否應(yīng)用于臨床提供參考。[方法]第一部分:上頜擴(kuò)弓動(dòng)物模型的建立。實(shí)驗(yàn)選用24只4周齡SD大鼠,完全隨機(jī)分為實(shí)驗(yàn)組與對(duì)照組。0.016英寸澳絲彎置成二眼圈簧擴(kuò)弓裝置,末端插入實(shí)驗(yàn)組大鼠第一、二磨牙牙間隙內(nèi),光固化復(fù)合樹(shù)脂將擴(kuò)弓裝置固定于大鼠磨牙舌側(cè),所有大鼠腭中縫兩側(cè)植入種植釘,以便觀察腭中縫擴(kuò)展量,通過(guò)雙側(cè)第一磨牙頰尖以及兩顆種植體之間的距離變化觀察擴(kuò)弓效果。第二部分:降鈣素對(duì)大鼠上頜擴(kuò)弓早期效果影響的研究,建模方法與第一部分相同。24只4周齡SD大鼠,隨機(jī)分為實(shí)驗(yàn)組與對(duì)照組。實(shí)驗(yàn)組大鼠每天15IU/kg濃度降鈣素腹腔注射,對(duì)照組大鼠等量生理鹽水每天腹腔注射。通過(guò)雙側(cè)第一磨牙頰尖以及兩側(cè)微種植釘間距離的變化,觀察降鈣素對(duì)擴(kuò)弓效果的影響。第三部分:降鈣素對(duì)大鼠上頜擴(kuò)展初期腭中縫改建影響的研究,對(duì)第二部分實(shí)驗(yàn)擴(kuò)弓結(jié)束的24只大鼠,兩側(cè)第一磨牙連線處制作組織脫礦切片,進(jìn)行相關(guān)染色,觀察腭中縫骨質(zhì)改建情況。[結(jié)果]1.兩眼圈簧矯治器以及種植釘植入會(huì)對(duì)大鼠健康產(chǎn)生一定影響(P0.05);實(shí)驗(yàn)組大鼠雙側(cè)第一磨牙頰尖最外側(cè)距離以及雙側(cè)種植釘距離較對(duì)照組明顯增寬(P0.05)。2.降鈣素的作用下實(shí)驗(yàn)組大鼠雙側(cè)第一磨牙頰尖最外側(cè)距離以及雙側(cè)種植釘距離較對(duì)照組明顯增寬(P0.05)。3.組織形態(tài)學(xué)觀察到實(shí)驗(yàn)組大鼠(注射降鈣素)腭中縫區(qū)域成骨細(xì)胞較對(duì)照組多(P0.05),腭中縫軟骨寬度亦明顯增加,骨質(zhì)改建更加明顯。[結(jié)論]1.兩眼圈簧擴(kuò)弓矯治器成功建立了大鼠上頜擴(kuò)弓模型。2.腭中縫兩側(cè)植入微種植釘是一種簡(jiǎn)便、可靠、準(zhǔn)確觀測(cè)腭中縫實(shí)際擴(kuò)展量的方法。3.降鈣素能夠促進(jìn)上頜擴(kuò)弓的效果。4.降鈣素在腭中縫擴(kuò)展初期能促進(jìn)骨質(zhì)的改建。
[Abstract]:[objective] in orthodontic treatment, maxillary arch dilatation can effectively correct the malfunction caused by dental arch underdevelopment. Deformity, however, the effect of maxillary arch expansion is unstable due to various factors, and there is a certain recurrence rate. Calcitonin is a polypeptide hormone secreted by paraflosa cells of thyroid gland, which can regulate calcium ion flow, promote calcium salt deposition and bone remodeling. However, the effect of calcitonin on maxillary arch dilatation has not been studied. The purpose of this study was to investigate the effect of calcitonin on maxillary arch dilatation in rats by establishing a rat model of maxillary arch dilatation in order to provide a reference for clinical application in the future. Methods: the first part: the establishment of maxillary arcuate animal model. Twenty-four 4-week-old SD rats were randomly divided into two groups: the experimental group and the control group. The 0.016 inch Australian wire was bent into a two-ring Reed reaming device, and the end was inserted into the first and second molar space of the experimental group. Light cured composite resin fixed the arch expansion device on the lingual side of the rat molars, and implant pins were implanted on both sides of the palatal suture in all rats to observe the amount of expansion of the palatal suture. The effect of arch expansion was observed by the distance between the buccal tip of bilateral first molar and the two implants. The second part: the effect of calcitonin on the early stage of maxillary arch dilatation in rats. The modeling method was the same as that in the first part. 24 4-week-old SD rats were randomly divided into experimental group and control group. Calcitonin was injected intraperitoneally at 15IU/kg concentration in the experimental group and intraperitoneally injected with the same amount of saline in the control group. The effect of calcitonin on arch expansion was observed by the change of distance between buccal tip and bilateral microimplant nail of bilateral first molar. The third part: the effect of calcitonin on palatal suture remodeling in rats at the beginning of maxillary expansion. Tissue demineralization sections were made at the line of the first molar on both sides of 24 rats at the end of the second part of the experimental expansion. To observe the remodeling of palatine suture bone. [result] 1. The distance of bilateral first molar buccal tip and bilateral implant nail in the experimental group was significantly wider than that in the control group (P0.05). The distance between the buccal tip of bilateral first molar and the distance of bilateral implant nail in the experimental group was significantly wider than that in the control group (P0.05) under the action of calcitonin (P0.05). Histomorphologic observation showed that there were more osteoblasts in the palatine suture region in the experimental group than in the control group (P0.05). The width of palatine raphe cartilage and bone remodeling were also significantly increased in the experimental group (P0.05). [conclusion] 1. The rat maxillary arch expansion model was successfully established with two eye ring Reed reaming appliance. 2. It is a simple, reliable and accurate method to observe the actual expansion of palatine suture by implanting microimplant nail on both sides of palate suture. Calcitonin can promote the effect of maxillary arch expansion. 4. Calcitonin promotes bone remodeling at the beginning of palatine suture expansion.
