上頜前牙區(qū)單顆種植體周圍軟組織的美學(xué)評價

【摘要】:目的 對上頜單個前牙缺失患者,采用軟組織水平種植體與骨組織水平種植體修復(fù)后的軟組織美學(xué)效果進行觀察和分析,用紅色美學(xué)指數(shù)(Pink Esthetic Score,PES)評價兩類種植體在上頜前牙區(qū)的應(yīng)用范圍、成功率、美學(xué)效果及其他相關(guān)因素。 方法 對2011年1月至2013年7月在鄭州大學(xué)口腔醫(yī)院種植科接受種植治療的85例上頜前牙區(qū)單牙缺失患者病例資料進行回顧分析。共植入種植體85顆,其中軟組織水平種植體組41例,男22例,女19例,年齡39.1±2.2歲,植入41顆ITI種植體;骨組織水平種植體組44例,男28例,女16例,年齡36.8±1.8歲,植入44顆Ankylos種植體。修復(fù)后3-6個月隨訪,觀察兩組病例在戴牙時及隨訪時的口內(nèi)正面照片,用PES指數(shù)對種植體周圍軟組織進行美學(xué)評價,分析各自變化情況。 結(jié)果 使用PES指數(shù)評價種植體周圍軟組織,軟組織水平組指數(shù)由戴牙時(基線期)的7.24±2.23上升到隨訪時(隨訪期)的9.62±2.06,差異有統(tǒng)計學(xué)意義,種植體周圍黏膜炎2例,無種植體周圍炎。骨組織水平組指數(shù)由基線期的7.68±2.19上升到隨訪期的10.14±2.01,差異有統(tǒng)計學(xué)意義,種植體周圍黏膜炎3例,種植體周圍炎1例;期和隨訪期兩組的PES各單項指數(shù)、PES總和的差異均無統(tǒng)計學(xué)意義,兩組種植體留存率均為100%。 結(jié)論 軟組織水平種植體與骨組織水平種植體在修復(fù)體戴入后的半年內(nèi),上頜前牙區(qū)單牙種植體周圍軟組織的美學(xué)效果均有明顯改善,臨床觀察結(jié)果顯示,二者在前牙區(qū)均可獲得滿意的臨床療效。對于前牙區(qū)種植修復(fù),骨組織水平種植體似乎比軟組織水平種植體更易獲得良好的美學(xué)效果。種植體周圍軟組織的美學(xué)效果受多因素影響,,為了達到較好的美學(xué)效果,應(yīng)該嚴格選擇適應(yīng)證、制定合理的外科手術(shù)方案及修復(fù)方式。
[Abstract]:Objective to observe and analyze the aesthetic effect of soft tissue implant and bone tissue implant in patients with maxillary single anterior tooth loss, and to use the red aesthetic index (Pink Esthetic Score,). PES) to evaluate the application range, success rate, aesthetic effect and other related factors of two kinds of implants in maxillary anterior teeth. Methods from January 2011 to July 2013, the data of 85 patients with single tooth loss in maxillary anterior teeth who were treated with implant therapy in Department of Stomatology, Zhengzhou University were retrospectively analyzed. A total of 85 implants were implanted, including 41 cases in the soft tissue level implant group, 22 males and 19 females, aged 39.1 鹵2.2 years, and 41 ITI implants were implanted. Bone tissue level implant group (44 cases, male 28, female 16, age 36.8 鹵1.8 years) was implanted with 44 Ankylos implants. Three to six months after the restoration, the patients in the two groups were followed up. The intraoral positive photographs of the two groups were observed, and the aesthetic evaluation of the soft tissue around the implants was carried out with PES index, and the changes of each group were analyzed. Results the PES index was used to evaluate the soft tissue around implants. The index of soft tissue level increased from 7.24 鹵2.23 to 9.62 鹵2.06 at the time of wearing teeth (baseline period), and the difference was statistically significant. There were 2 cases of periimplant mucositis, no periimplant inflammation. The index of bone tissue level increased from 7.68 鹵2.19 in the baseline period to 10.14 鹵2.01in the follow-up period. The difference was statistically significant. There were 3 cases of periimplant mucositis and 1 case of peri-implant inflammation. There was no significant difference in PES index and PES summation between baseline and follow-up period. The retention rate of implants in both groups was 100. Conclusion the aesthetic effects of soft tissue implant and bone tissue implant were significantly improved within half a year after the restoration. The clinical observation showed that the aesthetic effect of the soft tissue around the single dental implant in the maxillary anterior dental region was significantly improved. Both of them could obtain satisfactory clinical effect in anterior teeth. For anterior dental implants, bone tissue level implants seem to have better aesthetic effects than soft tissue implants. The aesthetic effect of soft tissue around implants is affected by many factors. In order to achieve better aesthetic effect, it is necessary to strictly select indications, formulate reasonable surgical procedures and repair methods.
