非綜合征性腭裂兒童語言理解與表達(dá)能力的研究

【摘要】:目的觀察非綜合征性腭裂(NSCP)兒童的語言理解與表達(dá)能力與同齡正常兒童相比是否存在差異,并分析可能的發(fā)生機(jī)制。方法對(duì)54例3~8歲居住于城市地區(qū)的NSCP兒童,按養(yǎng)育者受教育程度,以初中畢業(yè)為標(biāo)準(zhǔn)分為2組,養(yǎng)育者受教育程度初中畢業(yè)及以上的兒童記為NSCP-A組,養(yǎng)育者受教育程度初中畢業(yè)以下的兒童記為NSCP-B組,分別與相應(yīng)的正常兒童(匹配年齡、性別、居住地和養(yǎng)育者受教育水平,記為正常對(duì)照A、B組),進(jìn)行標(biāo)準(zhǔn)化語言能力測(cè)試(包括整體語言、語言理解、語言表達(dá)、語義和句法5個(gè)項(xiàng)目),觀察NSCP組與正常對(duì)照組的測(cè)試結(jié)果是否存在差異。結(jié)果 NSCP組與正常對(duì)照組間的標(biāo)準(zhǔn)化語言能力測(cè)試得分的差異均有統(tǒng)計(jì)學(xué)意義(P0.05);NSCP組在語言表達(dá)和句法應(yīng)用能力項(xiàng)目上的得分均低于正常對(duì)照組;NSCP-B組在句法項(xiàng)目上的得分均值低于常模正常值。結(jié)論與正常兒童相比,NSCP兒童對(duì)復(fù)雜語言結(jié)構(gòu)的分辨和運(yùn)用能力相對(duì)不足;養(yǎng)育者的受教育水平對(duì)患兒的語言能力有明顯影響,養(yǎng)育者較高的受教育水平可以有效地提高NSCP患兒的語言表達(dá)和句法能力。因此,對(duì)NSCP兒童的序列治療,應(yīng)重視對(duì)患兒復(fù)雜語言結(jié)構(gòu)運(yùn)用能力的培養(yǎng),以提高該群體的整體預(yù)后水平。
[Abstract]:Objective to investigate the differences in language comprehension and expression between children with non-syndromic cleft palate (NSCP) and normal children of the same age, and to analyze the possible mechanism. Methods 54 NSCP children aged 3 to 8 years living in urban area were divided into two groups according to the education level of the nurturer, and the children who graduated from junior middle school and above were counted as the NSCP-A group. Children below junior high school were classified as NSCP-B group and matched with normal children (matching age, sex, place of residence and education level of nurseries). The results of standardized language proficiency tests (including global language, language understanding, language expression, semantic and syntax) were recorded as normal control group B and observed whether there were differences between the NSCP group and the normal control group. Results the scores of standardized language proficiency test between NSCP group and normal control group were significantly different (P0.05) the scores of language expression and syntactic application ability in NSCP group were lower than those in normal control group NSCP-B group in syntactic items. The average score is lower than the normal value of the norm. Conclusion compared with normal children, NSCP children have less ability to distinguish and use complex language structures, and the educational level of nurturers has a significant effect on their language ability. The higher education level of nurturers can effectively improve the language expression and syntactic ability of children with NSCP. Therefore, in order to improve the overall prognosis of NSCP children, attention should be paid to the training of their ability to use complex language structures.
