【摘要】：目的:觀察干細胞因子(stem cell factor,SCF)在慢性根尖周病患者肥大細胞(mast cells,MCs)中的表達情況,探討SCF陽性MCs在根尖周病發(fā)病機制及根尖周纖維組織的形成,尤其在根尖周囊腫纖維組織形成中的作用。方法:選擇2011年7月~2013年6月期間在暨南大學荔灣口腔教學醫(yī)院臨床診斷為根尖囊腫或根尖肉芽腫并接受根尖刮治手術的病例30例,其中根尖肉芽腫15例,根尖囊腫15例。正常對照組20例,取自因正畸需要拔除的牙周健康的前磨牙。標本經10%甲醛固定48 h以上。組織標本經石蠟包埋、組織連續(xù)切片,HE染色,光學顯微鏡下觀察其組織學改變;采用免疫組織化學法,觀察各組標本組織中MCs數目及MCs脫顆粒情況;采用免疫熒光雙染色法,在熒光顯微鏡下觀察各組標本組織中tryptase-SCF雙陽性MCs數目。結果:免疫組織化學染色結果顯示,2組慢性根尖周病組織中MCs和脫顆粒MCs數量與正常對照組相比顯著增多(P0.01);根尖囊腫組的MCs和脫顆粒MCs的數量均明顯高于根尖肉芽腫組(P0.05)。免疫熒光雙染色結果顯示,2組慢性根尖周病組織中的tryptaseSCF雙陽性MCs密度與正常對照組相比顯著增多(P0.01);根尖囊腫組tryptase-SCF雙陽性MCs密度明顯高于根尖肉芽腫組(P0.05)。各組標本組織中免疫組織化學染色MCs密度與免疫熒光雙染色MCs密度無顯著差異(P0.05)。結論:Tryptase-SCF雙陽性肥大細胞在慢性根尖周病的炎癥損害,尤其在根尖囊腫的纖維組織形成中發(fā)揮重要作用。Tryptase-SCF雙陽性肥大細胞與慢性根尖周病炎癥損害的發(fā)生、進展及其持續(xù)存在有關。
[Abstract]:Objective: to observe the expression of stem cell factor (stem cell factor,SCF) in mast cells (mast cells,MCs) of patients with chronic periapical disease, and to investigate the pathogenesis of SCF positive MCs in periapical disease and the formation of periapical fibrous tissue. In particular, the role of periapical cysts in the formation of fibrous tissue. Methods: from July 2011 to June 2013, 30 cases of apical cyst or apical granuloma, including 15 cases of apical granuloma and 15 cases of apical cyst, were selected from Liwan Stomatological Teaching Hospital of Jinan University. 20 cases of normal control group were extracted from healthy premolars due to orthodontics. The specimens were fixed with 10% formaldehyde for more than 48 hours. Tissue specimens were embedded in paraffin wax, stained with HE and stained with HE, and their histological changes were observed under optical microscope. The number of MCs and MCs degranulation were observed by immunohistochemical method, and immunofluorescence double staining was used. The number of tryptase-SCF double positive MCs in tissues of each group was observed under fluorescence microscope. Results: the results of immunohistochemical staining showed that the number of MCs and degranulated MCs in the chronic periapical disease tissues of the two groups was significantly higher than that in the control group (P0.01), and the number of MCs and MCs in the apical cyst group was significantly higher than that in the apical granuloma group (P0.05). The results of double immunofluorescence staining showed that the density of tryptaseSCF double positive MCs in the tissue of chronic periapical disease was significantly higher than that in the control group (P0.01), and the density of tryptase-SCF double positive MCs in the apical cyst group was significantly higher than that in the apical granuloma group (P0.05). There was no significant difference in the density of immunohistochemical staining MCs and immunofluorescence double staining MCs in each group (P0.05). Conclusion the double positive mast cells play an important role in the inflammatory damage of chronic periapical disease, especially in the formation of fibrous tissue of the apical cyst. The occurrence of the inflammatory damage in chronic periapical disease and the occurrence of the double positive mast cells of Tryptase-SCF may play an important role in the pathogenesis of chronic periapical disease. Progress and its persistence are relevant.
