翼外肌在髁突矢狀骨折愈合中對髁突應(yīng)力分布作用的三維有限元研究

【摘要】:目的從生物力學(xué)角度分析髁突矢狀骨折(SFMC)愈合進(jìn)程中的應(yīng)力分布情況,為SFMC臨床治療提供依據(jù)。方法以三維有限元法建立包含雙側(cè)顳頜關(guān)節(jié)(TMJ)的下頜骨有限元模型,設(shè)左側(cè)為骨折側(cè),用0.1mm的薄層模擬骨折裂隙,建立髁突矢狀骨折模型,計算骨折不同愈合階段下頜骨整體的應(yīng)力分布狀況和骨折側(cè)髁突、關(guān)節(jié)盤表面應(yīng)力分布、骨折斷端位移的狀況。結(jié)果翼外肌牽拉時,Ⅱ型骨折髁突游離端在骨折當(dāng)天到自然愈合的第4周應(yīng)力和位移都增大,各觀測組均于第4~12周趨于平穩(wěn),髁突殘端在自然愈合過程中的應(yīng)力和位移較平穩(wěn);關(guān)節(jié)盤殘端骨折當(dāng)天到自然愈合的第4周應(yīng)力減小,第4~12周趨于平穩(wěn),位移趨于平穩(wěn);關(guān)節(jié)盤游離端骨折當(dāng)天到自然愈合的第4周應(yīng)力和位移都增大,第4~12周趨于平穩(wěn)。切斷翼外肌后,Ⅱ型骨折髁突游離端在骨折當(dāng)天到自然愈合的第4周應(yīng)力和位移都增大,第4~12周趨于平穩(wěn),髁突殘端在骨折當(dāng)天到愈合第4周的應(yīng)力和位移減小,第4~12周趨于平穩(wěn);關(guān)節(jié)盤殘端骨折當(dāng)天到自然愈合的第4周應(yīng)力減小,第4~12周趨于平穩(wěn),位移趨于平穩(wěn);關(guān)節(jié)盤游離端骨折當(dāng)天到自然愈合的第4周應(yīng)力和位移都增大,第4~12周趨于平穩(wěn)。結(jié)論翼外肌在髁突矢狀骨折愈合中對髁突應(yīng)力和位移有影響,在髁突矢狀骨折治療時應(yīng)保留翼外肌。
[Abstract]:Objective to analyze the stress distribution in the healing process of condylar sagittal fracture (SFMC) from the biomechanical point of view so as to provide evidence for the clinical treatment of SFMC. Methods the finite element model of mandible including bilateral temporomandibular joint (TMJ) was established by three dimensional finite element method. The left side was used as the fracture side, and the condylar sagittal fracture model was established by simulating fracture with thin layer of 0.1mm. The stress distribution of the mandible at different healing stages, the stress distribution of the lateral condyle, the surface stress of the articular disc, and the displacement of the broken end of the fracture were calculated.
