【摘要】：目的:研究短牙弓患者顳下頜關(guān)節(jié)的負(fù)荷以及短牙弓與顳下頜關(guān)節(jié)紊亂的關(guān)聯(lián)。方法:通過CT掃描獲得頭面部原始數(shù)據(jù),建立三維有限元模型,比較完整牙弓和短牙弓的顳下頜關(guān)節(jié)應(yīng)力和負(fù)荷;臨床上收集短牙弓病例,根據(jù)患者的修復(fù)意愿分成兩組(活動(dòng)義齒RPD組和短牙弓SDA組),同時(shí)收集完整牙列的患者作為對(duì)照組(全牙弓CDA組),比較3組患者的顳下頜關(guān)節(jié)紊亂相關(guān)體征。結(jié)果:有限元分析發(fā)現(xiàn)隨著牙弓的縮短,顳下頜關(guān)節(jié)應(yīng)力和負(fù)荷增大;臨床檢查發(fā)現(xiàn)短牙弓組顳下頜關(guān)節(jié)紊亂體征的陽性率增加,但3組之間沒有統(tǒng)計(jì)學(xué)差異。結(jié)論:短牙弓雖然不是顳下頜關(guān)節(jié)紊亂的危險(xiǎn)因素,但是要關(guān)注短牙弓患者顳下頜關(guān)節(jié)的變化。
[Abstract]:Objective: To study the temporomandibular joint load and the relationship between the short dental arch and temporomandibular joint disorder in patients with short dental arch.Methods: The original data of head and face were obtained by CT scanning, and a three-dimensional finite element model was established to compare the stress and load of temporomandibular joint between complete dental arch and short dental arch. Result: The stress and load of TMJ increased with the shortening of denture arch. The clinical examination showed that TMJ stress and load increased with the shortening of denture arch. Conclusion: Short dental arch is not a risk factor for temporomandibular disorder, but should be paid attention to the changes of temporomandibular joint in patients with short dental arch.
