【摘要】：目的針對上頜竇底剩余牙槽骨高度(residual bone height,RBH)2~3 mm的患者行上頜竇底外提升同期種植體植入術的病例研究報道較少。文中旨在評價應用上頜竇底外提升同期種植體植入術修復上頜后牙區(qū)嚴重骨量不足病例的臨床效果。方法回顧性分析2012年1月至2013年1月在常州市口腔醫(yī)院種植中心行上頜竇底外提升同期種植體植入術的42例患者的臨床資料,共植入85顆種植體。術前錐形束計算機體層掃描測量上頜后牙種植區(qū)剩余牙槽骨高度,其中RBH為2~3 mm的53枚種植體為試驗組,32顆RBH在4~5 mm的種植體為對照組。85枚種植體中單顆種植體為單冠組(19顆),≥2顆種植體為橋體組(66顆)。術中采用超聲骨刀行上頜竇底外提升同期種植體植入術,并植入自體骨+人工骨粉混合物行骨再生術,修復前測量種植體穩(wěn)定性系數(shù)(Implant stability Quotient,ISQ),術后2年測量牙槽嵴頂至上頜竇底的高度,隨訪24個月,觀察臨床療效。結果試驗組和對照組術后24個月骨提升高度、ISQ比較差異有統(tǒng)計學意義[(7.755±0.376)vs(6.658±0.415)mm;(74.444±1.462)vs(73.058±1.477),P0.05]。logistic回歸分析顯示術后24個月骨提升高度的OR值為1.248(95%CI:0.242~6.427)。種植體提升高度對種植體的穩(wěn)定性的影響無統(tǒng)計學意義(P=0.149)。試驗組的種植體存留率與對照組比較,差異無統(tǒng)計學意義(98.11%vs 100%,P=0.953)。單冠組種植體存留率與橋體組比較,差異無統(tǒng)計學意義(94.74%vs 100%,P=0.189)。術后24個月復查,試驗組、對照組種植體周圍骨均整合良好并成功負載,種植體頸部骨少量吸收,種植體穩(wěn)定。結論上頜竇外提升同期種植體植入術應用于上頜后牙區(qū)嚴重骨量不足的種植修復病例臨床效果滿意。
[Abstract]:Objective to study the patients with 2mm (residual bone height of residual alveolar bone in the maxillary sinus floor undergoing simultaneous implant implantation with exasperation of the maxillary sinus floor. The purpose of this study was to evaluate the clinical effect of simultaneous implant implantation in the repair of severe bone deficiency in maxillary posterior teeth. Methods from January 2012 to January 2013, the clinical data of 42 patients who underwent the simultaneous implant implantation in the implant center of Changzhou Stomatological Hospital were analyzed retrospectively. A total of 85 implants were implanted. The height of residual alveolar bone in maxillary posterior dental implant was measured by computer tomography of conical bundle before operation.
