應(yīng)用口腔手術(shù)顯微鏡探查和疏通下頜磨牙近中中央根管的臨床研究

【摘要】:目的:評(píng)價(jià)口腔手術(shù)顯微鏡用于下頜磨牙探查和疏通近中中央根管的臨床效果。方法:采集門(mén)診患者126人,下頜磨牙139顆。由3位高年資醫(yī)生分別在常規(guī)肉眼下和口腔手術(shù)顯微鏡下進(jìn)行下頜磨牙近中中央根管的探查和疏通,比較兩者成功率。對(duì)下頜磨牙近中中央根管口進(jìn)行定位測(cè)量,并采用偏移投照方法描記該根管走行特點(diǎn)。結(jié)果:在顯微鏡幫助下,所有下頜磨牙近中中央根管的探查和疏通成功率分別提高了11.5%和10.1%。下頜第一磨牙和第二磨牙近中中央根管的疏通率,分別由5.0%增加到15.0%,3.4%增加到了13.6%;而它們的探查成功率分別提高了12.5%和10.2%。所有近中中央根管口均位于髓底發(fā)育溝,46.2%靠近近中舌根管口,30.8%在兩根管口中間,23.1%靠近近中頰根管口。所有疏通的近中中央根管有45%與近中舌側(cè)根管融合,未發(fā)現(xiàn)獨(dú)立根尖孔類(lèi)型。結(jié)論:在口腔手術(shù)顯微鏡的幫助下,探查和疏通下頜磨牙近中中央根管的數(shù)量均有顯著提高。
[Abstract]:Objective: to evaluate the clinical effect of dental surgery microscope for probing and dredging proximal central root canal of mandibular molars. Methods: 126 outpatients and 139 mandibular molars were collected. Three senior doctors were used to explore and dredge the proximal central root canal of mandibular molars under conventional naked eye and oral surgery microscope, and the success rates were compared. The proximal central root canal orifice of mandibular molars was measured and the characteristics of the root canals were recorded by offset projection method. Results: with the help of microscope, the success rate of probing and dredging the proximal central root canal of all mandibular molars was increased by 11.5% and 10.1%, respectively. The osseous rate of the first mandibular molar and the second molar increased from 5.0% to 15.0% to 13.6%, respectively, and their successful rate of exploration increased by 12.5% and 10.2%, respectively, in the mandibular first molar and the second molar, in the proximal central root canal of the mandibular first molar and the second molar, and increased from 5.0% to 15.0% and 10.2%, respectively. All the proximal central root canal orifices were located in 46.2% of the developing sulcus of the pulp floor near 30.8% of the proximal tongue root canal orifice and 23.1% near the mesiobuccal root canal orifice between the two canal orifices. 45% of the proximal central canals were fused with the lateral root canals of the proximal tongue, and no independent apical foramen type was found. Conclusion: with the help of oral surgery microscope, the number of probing and clearing the proximal central root canal of mandibular molars is significantly increased.
