手術(shù)導(dǎo)航系統(tǒng)在口腔頜面外科中的應(yīng)用研究

【摘要】:目的: 探討手術(shù)導(dǎo)航系統(tǒng)在頜面外科手術(shù)中的應(yīng)用。 方法: 回顧浙江大學(xué)醫(yī)學(xué)院附屬第一醫(yī)院2013年8月至2014年2月,術(shù)中應(yīng)用BrainLAB手術(shù)導(dǎo)航系統(tǒng)的患者共12例,其中7例為頜骨腫瘤,包括造釉細胞瘤5例,髁狀突骨軟骨瘤1例,頜骨骨瘤1例,其余5例為異物侵入。所有病例術(shù)前完善薄層CT掃描并將數(shù)據(jù)保存為DICOM格式導(dǎo)入BrainLAB iPlan軟件系統(tǒng)后利用該軟件進行手術(shù)方案設(shè)計。術(shù)中在手術(shù)導(dǎo)航系統(tǒng)輔助下按術(shù)前設(shè)計方案完成手術(shù)。術(shù)后復(fù)查三維CT,確認手術(shù)效果。術(shù)后分別于第1、3月時隨訪,隨訪期間記錄患者術(shù)區(qū)切口情況、外觀和功能影響。 結(jié)果: 所有患者手術(shù)均順利按術(shù)前設(shè)計完成,術(shù)后患者面部外形良好,未出現(xiàn)明顯手術(shù)并發(fā)癥,功能無明顯影響。 結(jié)論: 手術(shù)導(dǎo)航系統(tǒng)的應(yīng)用,可以使手術(shù)方案的設(shè)計更加周密、直觀,提高手術(shù)精度、安全性及成功率。
[Abstract]:Objective: to explore the application of surgical navigation system in maxillofacial surgery. Methods: from August 2013 to February 2014, 12 patients with maxillary tumors, including 5 ameloblastoma, were treated with BrainLAB surgical navigation system. One case was condylar osteochondroma, one case was mandibular osteoma, and the other 5 cases were foreign body invasion. In all cases, the thin slice CT scan was improved before operation and the data was saved as DICOM format to be imported into the BrainLAB iPlan software system, and then the operation scheme was designed by using the software. The operation was performed according to the preoperative design with the aid of the surgical navigation system. Three-dimensional CTs were reexamined after operation to confirm the effect of operation. The patients were followed up at 1 and 3 months, respectively. The incision, appearance and function of the patients were recorded during the follow-up. Results: all the patients were successfully completed according to the preoperative design. The facial appearance of the patients after operation was good, no obvious complications occurred, and the function of the patients was not significantly affected. Conclusion: the application of surgical navigation system can make the design of operation plan more thorough and intuitionistic, and improve the accuracy, safety and success rate of operation.
