【摘要】：目的 通過臨床病例研究，對上頜美學(xué)區(qū)單顆牙即刻種植后即刻修復(fù)與延期修復(fù)的美學(xué)治療效果進(jìn)行初步的觀察和分析，探討臨時(shí)修復(fù)時(shí)機(jī)的選擇對最終修復(fù)的美學(xué)效果是否有顯著的影響。 方法 對20例上頜前牙單顆牙缺失的患者即刻植入種植體，實(shí)驗(yàn)組為即刻修復(fù)組共9例，于手術(shù)后戴入種植體支持式臨時(shí)冠，3~4個(gè)月后進(jìn)行永久冠修復(fù)；對照組為進(jìn)行臨時(shí)冠牙齦塑形的延期修復(fù)組共11例，于手術(shù)后戴入馬里蘭橋，3~4個(gè)月后行二期手術(shù)，，并戴入種植體支持式臨時(shí)冠進(jìn)行牙齦塑形，約2個(gè)月后行永久冠修復(fù)。采用種植體成功率、紅色美學(xué)評分（pink esthetic score，PES）和患者的滿意度對治療效果進(jìn)行評價(jià)。 結(jié)果 種植體成功率為95%，有1例在永久修復(fù)4個(gè)月后，唇側(cè)出現(xiàn)瘺管，種植體近中出現(xiàn)約2mm的骨吸收。紅色美學(xué)方面實(shí)驗(yàn)組PES總分均值為8.50±1.20，對照組PES總分均值為8.23±0.97，兩組之間的差別無統(tǒng)計(jì)學(xué)意義（P0.05）；無論是實(shí)驗(yàn)組還是對照組，修復(fù)3個(gè)月后的美學(xué)分值（實(shí)驗(yàn)組8.78±1.20，對照組8.64±0.92）要高于修復(fù)后當(dāng)天的分值（實(shí)驗(yàn)組8.22±1.20，對照組7.82±0.87），并且之間的差別有統(tǒng)計(jì)學(xué)意義（P0.05）；其中牙齦近遠(yuǎn)中齦乳頭的分值是最低的，其次是唇側(cè)牙齦高度，唇側(cè)牙齦曲線及牙根部突度、牙齦色質(zhì)的美學(xué)分值最高。滿意度方面，實(shí)驗(yàn)組患者對治療效果和治療過程的滿意率分別為88.89%和100%，對照組為81.82%和63.64%。 結(jié)論 即刻種植后采用臨時(shí)冠對牙齦進(jìn)行保存或重建都能取得較為理想的美學(xué)治療效果；即刻修復(fù)比臨時(shí)冠牙齦塑形的延期修復(fù)的美學(xué)治療效果要更好，但兩者之間的差別無統(tǒng)計(jì)學(xué)意義；修復(fù)3個(gè)月后的美學(xué)效果要好于修復(fù)后當(dāng)天，并且兩者之間的差別有統(tǒng)計(jì)學(xué)意義；即刻修復(fù)的患者對治療效果和治療過程的滿意率要高于延期修復(fù)者，但差別無統(tǒng)計(jì)學(xué)意義。
[Abstract]:Objective to observe and analyze the aesthetic effect of immediate and delayed restoration of a single tooth in maxillary aesthetic area by clinical case study. To explore whether the choice of temporary repair timing has a significant impact on the aesthetic effect of final restoration. Methods 20 cases of maxillary anterior teeth with a single missing tooth were immediately implanted with implants. The experimental group (n = 9) was treated with implant-supported temporary crown for 3 ~ 4 months. In the control group, 11 cases were treated with delayed restoration of temporary crown gingival shape. The patients were put into Maryland Bridge 3 ~ 4 months after operation for second stage operation, and implant supported temporary crown for gingival reconstruction, about 2 months later for permanent crown restoration. Implant success rate, red aesthetic score (pink esthetic scorepes) and patient satisfaction were used to evaluate the therapeutic effect. Results the success rate of implant was 95%. After permanent repair for 4 months, fistula appeared on the labial side and bone resorption of 2mm was found in the proximal implant. In terms of red aesthetics, the average value of PES in the experimental group was 8.50 鹵1.20, and that in the control group was 8.23 鹵0.97.The difference between the two groups was not statistically significant (P0.05), no matter in the experimental group or in the control group, there was no significant difference between the two groups (P0.05). The aesthetic scores of the experimental group (8.78 鹵1.20) and the control group (8.64 鹵0.92) were significantly higher than those of the control group (8.22 鹵1.20) and the control group (7.82 鹵0.87) (P0.05). The second is the height of the labial gingiva, the curve of the labial gingiva and the protruding degree of the root, and the aesthetic score of the color quality of the gingiva is the highest. The satisfaction rates of the patients in the experimental group were 88.89% and 100, respectively, and those in the control group were 81.82% and 63.64%, respectively. Conclusion the effect of immediate restoration with temporary crown on gingival preservation or reconstruction is better than that of delayed restoration of gingival shape with temporary crown, and the effect of immediate restoration is better than that of delayed restoration of gingival shape with temporary crown, and the effect of immediate restoration is better than that of delayed restoration of gingival shape with temporary crown. But the difference between the two is not statistically significant, the aesthetic effect after 3 months of restoration is better than that of the day after restoration, and the difference between the two is statistically significant. Patients with immediate repair were more satisfied with the efficacy and course of treatment than those with delayed repair, but the difference was not statistically significant.
