CGF促進(jìn)頜骨囊腫術(shù)后骨缺損修復(fù)的臨床研究

【摘要】:目的本實(shí)驗(yàn)中,將自體濃縮生長因子(CGF)單獨(dú)作為骨移植材料對頜骨缺損處進(jìn)行填充。與空白組進(jìn)行對比,在觀察期內(nèi)進(jìn)行CBCT(口腔錐體束CT)的拍攝,利用缺損處的骨密度值來評價(jià)手術(shù)后骨腔內(nèi)的骨質(zhì)重建。材料和方法36例囊腫術(shù)后骨缺損最大直徑在4.0cm以下,0.5cm以上的患者,按照填充方法的不同隨機(jī)分為兩組:18例患者在缺損處的骨腔內(nèi)填入CGF凝膠作為CGF組,18例術(shù)后骨腔內(nèi)不填入任何材料作為空白組。依據(jù)術(shù)中冰凍結(jié)果診斷為含牙囊腫的患者9例,根尖囊腫20例,鼻腭囊腫7例,上頜部囊腫17例,下頜部囊腫19例。囊腫直徑在(0.8~3.8)cm之間,平均為(2.51±1.87)cm。男性19例,女性17例。年齡10~47歲,平均年齡為26.42歲。所有患者于術(shù)后1、2、4個(gè)月復(fù)診并拍攝CBCT,使用Mimics Research 17.0圖像分析軟件對頜骨缺損區(qū)的骨密度值進(jìn)行測量,使用SPSS17.0統(tǒng)計(jì)軟件進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果1、同一組不同觀察時(shí)間在成骨效果上相比,CGF組和空白組之間有統(tǒng)計(jì)學(xué)差異。(P小于0.05)2、術(shù)后1月時(shí),CGF組與空白組結(jié)果間無顯著性差異(P大于0.05)術(shù)后2月時(shí),CGF組與空白組結(jié)果間有顯著性差異(P小于0.05)術(shù)后4月時(shí),CGF組與空白組結(jié)果間有顯著性差異(P小于0.05)結(jié)論本次實(shí)驗(yàn)研究結(jié)果支持了CGF的促成骨作用,并且可作為單一材料,應(yīng)用于骨缺損區(qū)域的修復(fù),加速新生骨的生成。在臨床上,為患者縮短了治病療程,減少了疾病愈合的痛苦。
[Abstract]:Objective in this experiment, autogenous concentrated growth factor (CGF) was used as bone graft material to fill the mandibular defect. Compared with the blank group, CBCT (oral pyramidal tract CT) was taken during the observation period. Bone mineral density (BMD) of the defect was used to evaluate the bone reconstruction in the bone cavity after operation. Materials and methods 36 patients with bone defect whose maximum diameter below 4.0cm were more than 0.5 cm after operation were analyzed. According to the different filling methods, two groups of 18 patients were randomly divided into two groups: 18 cases were treated with CGF gel in the bone cavity of the defect as the CGF group and 18 cases were treated as the blank group without any material filling in the bone cavity after operation. According to intraoperative frozen results, 9 cases were diagnosed as dental cysts, 20 cases as apical cysts, 7 cases as nasopalatine cysts, 17 cases as maxillary cysts, 19 cases as mandibular cysts. The mean diameter of cyst was (2.51 鹵1.87) cm. There were 19 males and 17 females. The average age was 26.42 years. All the patients were followed up for 2 and 4 months after operation, and CBCTs were photographed. The bone mineral density was measured by Mimics Research 17.0 image analysis software, and SPSS 17.0 statistical software was used for statistical analysis. Results 1. There was significant difference in osteogenic effect between CGF group and blank group at different observation time (P < 0.05). 2. There was no significant difference between CGF group and blank group at 1 month after operation (P > 0.05) and between CGF group and blank group at 2 months after operation. There was significant difference between CGF group and blank group at 4 months after operation (P < 0.05). Conclusion the results of this experiment support the effect of CGF on bone formation. It can be used as a single material to repair bone defect and accelerate the formation of new bone. Clinically, the course of treatment is shortened and the pain of healing is reduced.
