【摘要】：目的探討顳下頜關(guān)節(jié)骨關(guān)節(jié)病(TMJOA)患者在不同骨關(guān)節(jié)病分期髁突三維影像學(xué)特征的差異。方法以2005年馬緒臣等提出的骨關(guān)節(jié)病X線表現(xiàn)分期為標(biāo)準(zhǔn),將108例TMJOA患者(骨關(guān)節(jié)病組)的錐形束CT(CBCT)檢查結(jié)果分為4期,對(duì)照組為28例僅存在間隙改變的顳下頜關(guān)節(jié)紊亂病患者;評(píng)價(jià)TMJOA CBCT征象不同分期時(shí)髁突的矢狀面位置,并對(duì)2組的髁突水平角進(jìn)行比較。結(jié)果 TMJOA不同分期髁突矢狀面位置無(wú)統(tǒng)計(jì)學(xué)差異,但髁突水平角有統(tǒng)計(jì)學(xué)差異(F=3.872,P=0.005),進(jìn)一步兩兩分析表明,第1期髁突水平角低于第2和3期(P=0.027,P=0.000);第2期對(duì)照組髁突水平角低于第3期(P=0.004,P=0.047)。結(jié)論 TMJOA患者在不同分期時(shí)髁突矢狀面位置差異不明顯,但髁突水平角存在差異,該角度的變化對(duì)TMJOA的分期有一定提示意義。
[Abstract]:Objective to investigate the difference of three dimensional imaging features of condyle in different stages of temporomandibular osteoarthropathy (TMJOA). Methods 108 cases of TMJOA (osteoarthropathy group) were divided into 4 stages by conical beam CT (CBCT) according to the X-ray staging of osteoarthropathy proposed by Ma Ogata et al in 2005. The control group consisted of 28 patients with temporomandibular joint disorder with only interspace changes. The sagittal position of condyle in different stages of TMJOA CBCT signs was evaluated and the condylar horizontal angle was compared between the two groups. Results there was no significant difference in sagittal position of condyle in different stages of TMJOA, but there was significant difference in horizontal angle of condylar process (FF3. 872% P0. 005). The condylar horizontal angle of stage 1 was lower than that of stage 2 and 3 (P0. 027), and that of stage 2 of control group was lower than that of stage 3 (P0. 004 and P0. 047). Conclusion there is no significant difference in the sagittal position of condyle in different stages of TMJOA, but there are differences in the horizontal angle of condyle, and the change of this angle may be helpful to the staging of TMJOA.
