球帽與Locator式下無(wú)牙頜4種植體覆蓋義齒的三維有限元分析

【摘要】:目的:通過(guò)建立4種植體支持的下無(wú)牙頜覆蓋義齒的三維有限元模型,分析不同受力方式下Locator與球帽2種連接方式對(duì)下頜骨、種植體及基臺(tái)應(yīng)力分布的影響。方法:采集下無(wú)牙頜患者的錐形束CT(cone beam computerized tomography,CBCT)數(shù)據(jù),應(yīng)用Mimics、Gomagic Studio、SolidWorks軟件分別建立Locator與球帽2種連接方式種植覆蓋義齒的三維有限元模型,結(jié)合ANSYS軟件分析其在不同受力方式下頜骨、種植體及基臺(tái)的應(yīng)力分布情況。結(jié)果:(1)不同受力方式下,2種連接方式中種植體頸部周?chē)跋骂M骨正中舌側(cè)下緣區(qū)皮質(zhì)骨出現(xiàn)應(yīng)力集中,而最大等效應(yīng)力(von-Mises應(yīng)力)位于頜骨正中舌側(cè)下緣皮質(zhì)骨區(qū);(2)種植體的von-Mises應(yīng)力集中在種植體頸部及其與附著體基臺(tái)連接部;(3)同種受力方式下,Locator連接的以上應(yīng)力集中區(qū)的應(yīng)力均小于球帽連接。結(jié)論:(1)4種植體覆蓋義齒的最大von-Mises應(yīng)力位于下頜骨正中舌側(cè)下緣區(qū)皮質(zhì)骨,而非種植體頸部周?chē)べ|(zhì)骨;(2)Locator連接較球帽連接的種植覆蓋義齒的von-Mises更小,更有利于牙槽骨的生理健康和種植體的長(zhǎng)期存留。
[Abstract]:Objective: to establish a three-dimensional finite element model of 4 implant-supported subdenture overdentures, and to analyze the effect of locator and ball cap on the stress distribution of mandible, implant and abutment under different loading modes. Methods: the data of conical cone beam computerized tomphography were collected from patients with edentulous jaw. The 3D finite element models of implant-overdenture were established by using Mimicsl Gomagic Studio SolidWorks software. The mandible was analyzed under different loading modes by using ANSYS software. Stress distribution of implants and abutments. Results (1) stress concentration was observed in the cortical bone around the neck of the implant and in the lower margin of the median lingual side of the mandible under different stress modes. The von-Mises stress of the implant located in the cortical bone region of the middle lingual side of the maxilla is concentrated in the neck of the implant and its connection with the attachment base. The stress of the zone is less than that of the ball cap connection. Conclusion the maximum von-Mises stress of the implant overdenture is located in the cortical bone of the lower margin of the median lingual side of the mandible, and the von-Mises of the implant-overdenture is smaller than that of the implant-overdenture with the bone-cap connection, but not the cortical bone around the neck of the implant. It is more beneficial to the physiological health of alveolar bone and the long-term retention of implants.
