【摘要】：目的對靶控輸注異丙酚和異丙酚復(fù)合瑞芬太尼2種鎮(zhèn)靜方法在小兒埋伏多生牙拔除手術(shù)中的應(yīng)用進(jìn)行比較。方法將60例接受上前牙區(qū)埋伏多生牙拔除術(shù)的患兒隨機(jī)分為2組:異丙酚組(P組,n=30)和異丙酚復(fù)合瑞芬太尼組(PR組,n=30)。P組:滴定法靶控輸注異丙酚至改良OAA/S評分3分時進(jìn)行手術(shù)。PR組:靶控輸注瑞芬太尼目標(biāo)濃度為1 ng·mL-1,滴定法靶控輸注異丙酚至改良OAA/S評分3分時進(jìn)行手術(shù)。采用Houpt行為治療效果評分評估2組患兒術(shù)中配合情況,記錄2組患兒心率、血壓、血氧飽和度、呼吸頻率和Narcotrend指數(shù)(NI),以及相關(guān)并發(fā)癥、不良反應(yīng)及藥物輸注情況。結(jié)果 Houpt行為治療效果評分PR組優(yōu)于P組(P0.05)。血氧飽和度、呼吸頻率PR組低于P組(P0.05),心率、血壓及NI值2組無統(tǒng)計學(xué)差異(P0.05)。呼吸抑制和順行性遺忘的發(fā)生率,PR組高于P組(P0.05)。結(jié)論在小兒埋伏多生牙拔除術(shù)中,靶控輸注異丙酚和靶控輸注異丙酚復(fù)合瑞芬太尼鎮(zhèn)靜這2種方法都是安全有效的。在吸氧狀態(tài)下,靶控輸注異丙酚復(fù)合瑞芬太尼鎮(zhèn)靜優(yōu)于異丙酚鎮(zhèn)靜。
[Abstract]:Objective to compare the two sedation methods of target controlled infusion of propofol and propofol combined with remifentanil in the extraction of ambush supernumerary teeth in children. Methods Sixty children undergoing ambushed extraction of overlying teeth were randomly divided into two groups: propofol group (P group) and propofol combined with remifentanil group (PR group). Group P: target controlled infusion of propofol by titration to the modified OAA/S score of 3 points. Group PR: target controlled infusion of remifentanil was 1 ng ml ~ (-1), target controlled infusion of propofol by titration and modified OAA/S score 3 min. Houpt behavioral therapy score was used to evaluate the intraoperative cooperation, heart rate, blood pressure, blood oxygen saturation, respiratory frequency and Narcotrend index, as well as related complications, adverse reactions and drug infusion were recorded in the two groups. Results the score of Houpt behavioral therapy effect in PR group was better than that in P group (P 0.05). Blood oxygen saturation and respiratory rate in PR group were lower than that in P group (P 0.05). There was no significant difference in heart rate, blood pressure and NI value between two groups. The incidence of respiratory inhibition and antegrade amnesia in PR group was higher than that in P group (P 0.05). Conclusion both target-controlled infusion of propofol and target-controlled infusion of propofol combined with remifentanil sedation are safe and effective in the extraction of ambush supernumerary teeth in children. Target controlled infusion of propofol combined with remifentanil was superior to propofol sedation under oxygen uptake.
