新疆不同民族舌體鱗癌預(yù)后因素及不同治療模式療效分析

【摘要】:目的:探討影響新疆不同民族舌體鱗狀細胞癌患者預(yù)后的臨床病理因素及不同治療模式的治療效果。方法:選擇2003年1月至2011年12月期間在新疆醫(yī)科大學(xué)第一附屬醫(yī)院首治的經(jīng)病理確診且隨訪資料完整的舌體鱗癌病例共132例,應(yīng)用Kaplan-Meier法行臨床病理因素單因素分析及計算單純手術(shù)(S)、單純放療(R)、手術(shù)+放療(S+R)、手術(shù)+化療(S+C)、放療+化療(R+C)、手術(shù)+放療+化療(S+R+C)各組治療后累計生存率,組間差異采用log-rank檢驗。Cox回歸行多因素分析確立影響預(yù)后的獨立因素及獨立治療模式。結(jié)果;132例舌體鱗癌患者的平均生存時間為75.65個月,中位生存期為80.00個月,3年和、5年生存率分別為72.7%、59.8%。單因素分析顯示:Ⅰ-Ⅱ期S、R、S+R、S+C、S+R+C各組患者的3年生存率分別為85.7%、66.7%、96.8%、100%、81.8%,P=0.018。Ⅲ-Ⅳ期S、R、S+R、S+C、 R+C、S+R+C各組患者的3年生存率分別為37.5%、14.3%、92.3%、40.0%、14.3%66.7%,P=0.000。多因素分析顯示影響預(yù)后的獨立因素包括:病理分級、原發(fā)灶手術(shù)、局部復(fù)發(fā)、TNM分期和放療,同時手術(shù)+放療、手術(shù)+放療+化療是影響預(yù)后的獨立因素。結(jié)論:影響預(yù)后的獨立因素為病理分級、原發(fā)灶手術(shù)、局部復(fù)發(fā)TNM分期和放療。原發(fā)灶的徹底手術(shù),合理的頸部淋巴結(jié)清掃及降低局部復(fù)發(fā)能改善舌體鱗癌患者的預(yù)后。Ⅰ-Ⅱ期行手術(shù)治療或者手術(shù)為主的綜合治療療效較好,Ⅲ-Ⅳ期行以手術(shù)為主的綜合治療預(yù)后較好,其中手術(shù)+放療、手術(shù)+放療+化療是較好的治療模式,對于中晚期病例行單純放療、放療+化療預(yù)后差。
[Abstract]:Objective: to investigate the clinicopathological factors affecting the prognosis of tongue squamous cell carcinoma (SCC) patients from different nationalities in Xinjiang. Methods: a total of 132 cases of tongue squamous cell carcinoma diagnosed by pathology and followed up from January 2003 to December 2011 in the first affiliated Hospital of Xinjiang Medical University were selected. Kaplan-Meier method was used to analyze the clinicopathological factors and calculate the cumulative survival rate of each group. Multivariate analysis was performed by log-rank test. Cox regression analysis was used to determine the independent prognostic factors and the independent treatment model. Results the average survival time of 132 patients with tongue squamous cell carcinoma was 75.65 months, the median survival time was 80.00months, the 3-year and 5-year survival rates were 72.7% and 59.8 months, respectively.
