【摘要】：目的：目前對(duì)于普萘洛爾與類(lèi)固醇激素治療頜面部嬰幼兒血管瘤（infantilehemangiomas，IHs）療效與安全性的問(wèn)題上仍存在爭(zhēng)議。國(guó)內(nèi)外已經(jīng)有許多學(xué)者對(duì)于兩種藥物做過(guò)臨床對(duì)比研究。本文旨在收集并總結(jié)相關(guān)文獻(xiàn)，為臨床合理用藥提供詢證醫(yī)學(xué)證據(jù)。 材料和方法：計(jì)算機(jī)檢索Medline數(shù)據(jù)庫(kù)、Pubmed數(shù)據(jù)庫(kù)、Cochrane圖書(shū)館臨床對(duì)照研究資料庫(kù)及萬(wàn)方數(shù)據(jù)庫(kù)、中國(guó)知網(wǎng)數(shù)據(jù)庫(kù)。搜索國(guó)內(nèi)外2013年9月以前發(fā)表的單純應(yīng)用普萘洛爾及單純應(yīng)用類(lèi)固醇激素治療血管瘤對(duì)照研究，對(duì)所納入的研究提取有效數(shù)據(jù)，并運(yùn)用Cochrane協(xié)作網(wǎng)提供的RevMan5.0軟件對(duì)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)學(xué)分析。 結(jié)果：按照已制定的檢索策略，，納入單純應(yīng)用普萘洛爾及單純應(yīng)用類(lèi)固醇激素治療血管瘤對(duì)照研究9篇。采用固定效應(yīng)模型進(jìn)行分析，結(jié)果顯示普萘洛爾組在療效方面優(yōu)于類(lèi)固醇激素組（RR=1.94,95%CI：1.40,2.69，P0.0001）差異有統(tǒng)計(jì)學(xué)意義。對(duì)藥物不良反應(yīng)做統(tǒng)計(jì)學(xué)分析，采用固定效應(yīng)模型進(jìn)行分析，結(jié)果顯示普萘洛爾組在藥物不良反應(yīng)方面優(yōu)于類(lèi)固醇激素組（RR=0.42,95%CI：0.28，0.64，P0.0001）差異有統(tǒng)計(jì)學(xué)意義。在治療IHs并發(fā)潰瘍方面，暫無(wú)兩類(lèi)藥物的隨機(jī)對(duì)照研究，不過(guò)通過(guò)總結(jié)納入的3篇普萘洛爾治療IHs并發(fā)潰瘍的研究，證明普萘洛爾較其他傳統(tǒng)治療方式，在治療IHs并發(fā)潰瘍方面有一定優(yōu)勢(shì)。 結(jié)論：與單純應(yīng)用類(lèi)固醇激素相比，普萘洛爾可以作為一線藥物治療嬰幼兒血管瘤。
[Abstract]:Objective: to investigate the efficacy and safety of propranolol and steroid in the treatment of infantile hemangiomatosis (IHs) in infants with maxillofacial hemangioma. Many scholars at home and abroad have done clinical comparative research on the two drugs. The purpose of this paper is to collect and summarize the relevant literature and provide evidence for rational use of drugs in clinic. Materials and methods: the Medline database was searched by computer. To search for the control study on the treatment of hemangioma with propranolol alone and steroid hormone alone published before September 2013, and to extract valid data from the included study, The data are analyzed statistically by using RevMan5.0 software provided by Cochrane Cooperative Network. Results: according to the search strategy, 9 articles were included in the control study of propranolol alone and steroid hormone alone in the treatment of hemangioma. The results showed that the effect of propranolol group was better than that of steroid hormone group. The adverse drug reactions were analyzed statistically and the fixed effect model was used. The results showed that the propranolol group was better than the steroid hormone group in the adverse drug reaction. The difference was statistically significant (P 0.0001). In the treatment of IHs with ulcers, there is no randomized controlled study of two kinds of drugs. However, by summarizing the three studies of propranolol in the treatment of IHs complicated with ulcers, it is proved that propranolol is more effective than other traditional treatment methods. It has some advantages in the treatment of IHs complicated with ulcers. Conclusion: propranolol can be used as a first-line drug in the treatment of infantile hemangioma.
