【摘要】：背景:牙槽突裂植骨是目前常見的修復(fù)唇腭裂患者牙槽突骨缺損的方法,但是在國(guó)內(nèi),關(guān)于牙槽突裂植骨對(duì)上唇高度改變的研究報(bào)道較少。目的:測(cè)量牙槽突裂植骨后上唇及鼻翼基底的改變情況。方法:46例確診為先天性單側(cè)牙槽突裂的患者(平均年齡為9.85歲),這些患者均采用自體髂骨作為供骨源,平均植入骨量為2.98 cm~3。通過(guò)統(tǒng)一的照相方法,獲得患者的正面照、側(cè)面照及仰頭鼻底照。通過(guò)對(duì)患者面部固定位置的點(diǎn)進(jìn)行測(cè)量,對(duì)患者的上唇高度、上唇投影、鼻翼基底植骨前比例指數(shù)和植骨后比例指數(shù)進(jìn)行比較。結(jié)果與結(jié)論:隨訪時(shí)間為6個(gè)月,結(jié)果顯示,上唇高度5組測(cè)量數(shù)據(jù)均顯示牙槽突裂植骨后上唇高度顯著延長(zhǎng),但差異無(wú)顯著性意義。上唇投影測(cè)量數(shù)據(jù)顯示上唇投影后更加明顯,但差異無(wú)顯著性意義。鼻翼基底4組測(cè)量數(shù)據(jù)中,有3組數(shù)據(jù)顯示牙槽突裂植骨后鼻翼基底顯著抬高,且差異有顯著性意義。結(jié)果提示,在單側(cè)完全性牙槽突裂患者中,通過(guò)牙槽突裂植骨可抬高鼻翼基底及延長(zhǎng)上唇高度。
[Abstract]:Background: alveolar process bone grafting is a common method to repair alveolar process defects in patients with cleft lip and palate. Objective: to measure the changes of upper lip and alar base of alveolar fissure after bone grafting. Methods 46 patients with congenital unilateral alveolar process fissure (mean age 9.85 years old) were treated with autogenous iliac bone as donor bone with an average bone mass of 2.98 cm ~ (3). Through the unified photographic method, the patient's frontal, lateral and suprahead nose radiography were obtained. The height of upper lip, projection of upper lip, ratio index of alar base before bone grafting and ratio index after bone grafting were compared. Results and conclusion: the follow-up time was 6 months. The results showed that the height of upper lip in all 5 groups was significantly prolonged after alveolar process bone grafting, but there was no significant difference. The upper lip projection data showed that the upper lip projection was more obvious, but there was no significant difference. Of the 4 groups of measurement data of the alar base, there were three groups of data showing significant elevation of the base of the alar after alveolar fissure and bone grafting, and the difference was significant. The results suggest that in patients with unilateral complete alveolar fissure, alveolar process bone graft can elevate the base of nasal alar and prolong the height of upper lip.
