【摘要】：[目的]探究孕前6個月至孕早期3個月內(nèi)(圍孕期)父母的生活環(huán)境及職業(yè)有害因素暴露與單純型先天性小耳畸形發(fā)病的相關(guān)性。[方法]采用病例對照研究設(shè)計,對2015年6月—2016年6月期間于上海交通大學(xué)醫(yī)學(xué)院附屬第九人民醫(yī)院就診的0~10歲單純型先天性小耳畸形患者(病例組)及同年齡段因外傷、發(fā)熱或急診就診患兒(對照組)的父母進行面對面訪談和問卷調(diào)查,收集其社會人口學(xué)以及圍孕期生活環(huán)境、生活習(xí)慣、職業(yè)接觸等資料,將獲得的數(shù)據(jù)進行單因素卡方檢驗和非條件多因素logistic回歸分析。[結(jié)果]最終獲得124例病例和126例對照的問卷資料。單因素分析發(fā)現(xiàn):圍孕期居住地附近(≤3 km2)有排放污水、固體廢物和空氣污染物的工業(yè)生產(chǎn)企業(yè),圍孕期居住地室內(nèi)裝修或添置新家具,母親圍孕期職業(yè)環(huán)境中的重金屬暴露、染發(fā)燙發(fā)、被動吸煙以及父親圍孕期重金屬和有機溶劑的職業(yè)有害因素暴露皆可能是先天性小耳畸形的危險因素(均P0.05)。非條件多因素logistic回歸分析的結(jié)果顯示:母親文化程度低(OR=0.737,95%CI:0.545~0.996),圍孕期室內(nèi)裝修(OR=4.546,95%CI:1.668~12.390),圍孕期母親被動吸煙頻率高(OR=1.420,95%CI:1.059~1.903)以及父親職業(yè)環(huán)境中重金屬暴露(OR=2.880,95%CI:1.036~8.007)均可能是小耳畸形發(fā)病的相關(guān)危險因素。[結(jié)論]圍孕期多種環(huán)境和職業(yè)有害因素暴露對小耳畸形發(fā)病的影響值得關(guān)注和進一步研究。
[Abstract]:[objective] to investigate the relationship between exposure of parents' living environment and occupational harmful factors to simple congenital microauricular malformation from 6 months before pregnancy to 3 months after pregnancy. [methods] A case-control study was used to study the patients (case group) with simple congenital microauricular malformation (age group) and trauma in the same age group who were admitted to the Ninth people's Hospital affiliated to the School of Medicine of Shanghai Jiaotong University from June 2015 to June 2016. Parents of children with fever or emergency (control group) received face-to-face interviews and questionnaires to collect data on social demography, living environment, living habits and occupational contacts during pregnancy. The obtained data were analyzed by single factor chi-square test and non-conditional multivariate logistic regression analysis. [results] the data of 124 cases and 126 controls were obtained.
