葡萄糖定量法比較五種根管封閉劑的封閉性能

【摘要】:根管治療失敗的最主要原因是根管充填后出現(xiàn)的微滲漏。本實驗中所檢測的5種根管封閉劑是目前臨床上較為代表性的封閉劑,主要是氧化鋅丁香油類封閉劑、樹脂類封閉劑及硅酮類封閉劑。目前為止還沒有對這5種封閉劑進行過根管充填后微滲漏的對比研究。 目的:通過葡萄糖定量檢測方法,檢測比較5種根管封閉劑—EZ Fill、AHPlus、2Seal、Cortisomol、GuttaFlow的根管充填后出現(xiàn)的微滲漏量。 材料方法:選取62顆離體單根管下頜前磨牙隨機分為5組,每組包含10顆牙齒,另設(shè)陰性對照組和陽性對照組各6顆。以機用Protaper配合5.25%次氯酸鈉溶液預(yù)備至F3,,然后以17%EDTA和5.25%次氯酸鈉交替沖洗。分別以EZ Fill、AH Plus、2Seal封閉劑和熱牙膠垂直加壓法充填10顆;GuttaFlow根管充填技術(shù)充填10顆;Cortisomol封閉劑和熱牙膠垂直加壓各充填16顆(其中6顆作為陰性對照組);剩余6顆僅用熱牙膠垂直加壓充填作為陽性對照組。將實驗組和陽性組牙體表面(除根尖孔外2mm)涂兩層不同顏色的指甲油,陰性組包括根尖孔在內(nèi)的所有牙面涂兩層指甲油,其后建立葡萄糖定量檢測模型,并在1、2、4、6及8w采用葡萄糖氧化酶比色法檢測滲漏出的葡萄糖濃度,并將所得數(shù)據(jù)用Excel錄入計算機,spss17.0統(tǒng)計軟件進行統(tǒng)計學(xué)處理與分析比較各組樣本在各時間段的微滲漏量。 結(jié)果:陰性對照組在各時間段均未檢測到葡萄糖的滲出;陽性對照組在1w即檢測到比較高濃度的葡萄糖,且表現(xiàn)出快速的增加趨勢。在1w時Cortisomol組微滲漏數(shù)值明顯大于AH Plus組,且有顯著性差異(P<0.05),2w和4w時微滲漏數(shù)值明顯大于EZ Fill、AH Plus、2Seal與GuttaFlow組,且有非常顯著性差異(P<0.001),而EZ Fill、AH Plus、2Seal與GuttaFlow組之間的微滲漏量之間無顯著性差異(P>0.05)。在6w和8w時AH Plus組的微滲漏數(shù)值明顯小于EZ Fill、2Seal、Cortisomol與GuttaFlow組,且有顯著性差異(P<0.05),同時,EZ Fill、2Seal、Cortisomol與GuttaFlow組之間微滲漏數(shù)值之間無統(tǒng)計學(xué)差異。隨著時間推移5種根管封閉劑微滲漏的葡萄糖量平穩(wěn)升高。 結(jié)論:本實驗結(jié)果顯示AH Plus的早期的封閉性能與EZ Fill、2Seal和GuttaFlow封閉劑無明顯差異,晚期優(yōu)于其他四組封閉劑;Cortisomol早期微滲漏明顯大于其他各組,較晚時其封閉性與EZ Fill、2Seal和GuttaFlow組無明顯差異,與AH Plus租比較微滲漏有明顯差異;而EZ Fill、2Seal和GuttaFlow組間在各階段均無明顯差異。
[Abstract]:The main reason for the failure of root canal therapy is the microleakage after root canal filling. The 5 kinds of root canal sealants detected in this experiment are the representative sealants in clinic at present, which are mainly zinc oxide clove oil sealants, resin sealants and silicone sealants. So far, there has not been a comparative study of microleakage after root canal filling with these five sealants. Objective: to detect and compare the microleakage of 5 root canal sealants-EZ Fillus AHPlus2 Sealtisomolus Cortisomolus GuttaFlow after root canal filling by quantitative detection of glucose. Materials: 62 single root canals mandibular premolars were randomly divided into 5 groups, each containing 10 teeth, and the negative control group and the positive control group each with 6 teeth. Mechanical Protaper and 5.25% sodium hypochlorite solution were used to prepare F _ 3 and then flushed alternately with 17TA and 5.25% sodium hypochlorite. 10 GuttaFlow root canal fillings were filled with EZ Fillon AH Plust2Seal sealant and hot gum vertical compression technique respectively. 10 Cortisomol sealants and 16 hot gum vertical compression were filled respectively (6 as negative control group and 6 as negative control group); the remaining 6 were filled with heat only. Gum vertical compression filling was used as the positive control group. Two layers of nail polish of different colors were applied on the surfaces of the experimental group and the positive group (except for the apical foramen), and all the teeth in the negative group, including the apical foramen, were coated with the two layers of nail polish, and the glucose quantitative detection model was established. Glucose concentration was detected by glucose oxidase colorimetry at 1w and 8w, and the data were recorded in the computer software SPSS 17.0 for statistical analysis and comparison of microleakage in each time period. Results: no exudation of glucose was detected in the negative control group and a relatively high concentration of glucose was detected in the positive control group at 1 week and showed a rapid increase trend. The microleakage value of Cortisomol group was significantly higher than that of AH Plus group at 1 week, and the microleakage value of EZ Fillar AH Plus2Seal group was significantly higher than that of EZ Fillar AH Plus2Seal group and GuttaFlow group at 4 weeks and P < 0.05 respectively. There was significant difference (P < 0. 001), but there was no significant difference in microleakage between EZ Fillus AH Plus2 Seal and GuttaFlow group (P > 0. 05). The microleakage value of AH Plus group at 6 and 8 weeks was significantly lower than that of EZ Filler2SealCortisomol group and GuttaFlow group (P < 0.05), and there was no significant difference between EZ Fillering2SealalCortisomol group and GuttaFlow group. The glucose content of microleakage of 5 root canal sealants increased steadily over time. Conclusion: the results showed that the early sealing performance of AH Plus was not significantly different from that of EZ Fill2 Seal and GuttaFlow sealants, and the early microleakage of AH Plus was significantly higher than that of other four sealants. There was no significant difference between EZ Fillering2Seal group and EZ Filler2 Seal group, but there was significant difference with AH Plus rent group in microleakage, but there was no significant difference between EZ Fill2 Seal group and GuttaFlow group in all stages.
