【摘要】：目的 比較無牙頜患者種植體支持式覆蓋義齒修復與種植固定義齒修復后的臨床療效和患者滿意度，為臨床醫(yī)生提供指導。 方法 對37例ITI、Zimmer、Osstem等種植系統(tǒng)修復的無牙頜患者進行1年的臨床觀察。采用臨床檢查、X射線觀測和患者主觀評分等來評價種植體與骨組織結合成功率，修復體使用情況，種植體周圍骨吸收情況，并進行種植治療后的滿意度調(diào)查。 結果 種植覆蓋義齒修復患者（A組）的92顆種植體和種植固定義齒修復患者（B組）的141顆種植體均形成良好的骨結合，種植體周圍無明顯骨吸收，牙周健康良好，A組修復體成功率96.89%，B組修復體成功率98.03%。A組和B組在種植治療后的滿意度在咀嚼、固位、舒適、發(fā)音方面無明顯差異，在方便和美觀方面存在明顯差異。 結論 對納入的37例無牙頜患者進行種植覆蓋義齒與固定義齒臨床療效對比研究得出結論： 1、無牙頜患者進行種植覆蓋義齒與固定義齒修復成功率高。 2、無牙頜患者種植覆蓋義齒與固定義齒修復后短期內(nèi)種植體周圍軟組織健康良好。 3、無牙頜患者種植覆蓋義齒修復后在患者總體滿意度及美觀方面優(yōu)于固定義齒修復。 4、無牙頜患者種植固定義齒修復后在患者滿意度方便性方面顯著優(yōu)于覆蓋義齒。 醫(yī)生在接診無牙頜患者時需要與患者耐心仔細的溝通，，掌握患者的就診目的和對義齒的期望值及患者的口腔條件，經(jīng)濟狀況，再結合種植覆蓋義齒與固定義齒修復的特點，合理引導患者選擇合適的種植修復治療方案。
[Abstract]:Purpose To compare the clinical efficacy and patient satisfaction of implant-supported overdenture and implant fixed denture in patients with edentulous jaw, and to provide guidance for clinicians. Method The clinical observation of 37 patients with edentulous jaw repaired by implant system such as ITII Zimmern Osstem for 1 year was carried out. The success rate of implant and bone tissue bonding, the use of prosthesis and the bone resorption around the implant were evaluated by clinical examination and subjective score of patients, and the satisfaction of implant treatment was investigated. Result 92 implants of implant-overdenture group (group A) and 141 implants of implant-fixed denture group (group B) all formed a good osseous bond, and there was no obvious bone resorption around the implants. The success rate of prosthesis in group A and group B was 98.03.A and B respectively. There was no significant difference in mastication, retention, comfort and pronunciation between group A and group B in terms of convenience and beauty. Conclusion The clinical effects of implant overdenture and fixed denture were compared in 37 patients with edentulous jaw. 1. The success rate of implant overdenture and fixed denture was high in patients with edentulous jaw. 2. The soft tissues around implants and fixed dentures in patients with edentulous maxillary implants were in good health in a short period of time. 3. Implant overdenture was superior to fixed denture in overall satisfaction and beauty. 4. The satisfactory convenience of implant fixed denture in edentulous patients was better than that of overdenture. Doctors need to communicate with the patients patiently and carefully when receiving patients with edentulous jaw patients, to grasp the purpose of the patient's visit, the expected value of the denture, the condition of the patient's mouth, and the economic condition, and then combine the characteristics of the implant overdenture with the fixed denture. The patients should be guided to select the appropriate implant repair therapy.
