【摘要】：【目的】探討機(jī)用鎳鈦器械對(duì)牙根管治療患兒應(yīng)激反應(yīng)及治療效果的影響,分析其可能作用機(jī)制�！痉椒ā窟x擇2015年3月-2017年3月行根管治療術(shù)乳牙牙髓病或根尖病患兒60例(60顆牙)為研究對(duì)象,采用隨機(jī)數(shù)字表法分為觀察組和對(duì)照組各30例(30顆牙),對(duì)照組采用手用鎳鈦擴(kuò)大挫,觀察組采用機(jī)用鎳鈦器械,比較兩組患兒根管預(yù)備效果、應(yīng)激反應(yīng)、疼痛程度、填充效果及遠(yuǎn)期療效等指標(biāo)。【結(jié)果】觀察組根管根治術(shù)患兒根管預(yù)備時(shí)間明顯短于對(duì)照組(P0.01),根管偏移等根管形態(tài)變化發(fā)生率6.67%,明顯低于對(duì)照組26.67%(P0.05);齦溝液腫瘤壞死因子-α(TNF-α)、白細(xì)胞介素-6(IL-6)、白細(xì)胞介素-1β(IL-1β)等含量明顯低于對(duì)照組(P0.05,P0.01);根管預(yù)備中、根管預(yù)備后疼痛率均明顯低于對(duì)照組(10.00%vs 33.33%,3.33%vs 23.33%,P0.05);術(shù)后1周,觀察組患兒恰填率93.33%,明顯高于對(duì)照組70.00%(P0.05);隨訪6個(gè)月,觀察組根管術(shù)成功率90.00%,明顯高于對(duì)照組66.67%(P0.05)�！窘Y(jié)論】機(jī)用鎳鈦器械有助于緩解牙根管治療患兒疼痛程度,提高臨床療效,可能與抑制手術(shù)應(yīng)激反應(yīng)有關(guān)。
[Abstract]:[objective] to investigate the effect of machine nickel titanium instrument on stress response and therapeutic effect of children with root canal therapy. [methods] 60 children with primary dental pulp or apical disease underwent root canal therapy from March 2015 to March 2017. The experimental group was randomly divided into two groups: the observation group (n = 30) and the control group (n = 30). The patients in the control group were divided into two groups: the control group (n = 30) and the control group (n = 30), and the control group (n = 30) were divided into two groups: the control group (n = 30) and the control group (n = 30). [results] the preparation time of root canal in the observation group was significantly shorter than that in the control group (P 0.01), and the incidence of root canal deviation was 6.67, which was significantly lower than that in the control group (26.67%, P 0.05), and the gingival crevicular fluid tumor was bad. In root canal preparation, the contents of dead factor- 偽 TNF- 偽, interleukin-6, interleukin-1 尾 and interleukin-1 尾 IL-1 尾 were significantly lower than those in control group. The rate of pain after root canal preparation was significantly lower than that in the control group (10.00 vs 33.333.33 vs 23.33P0.05), and at one week after operation, the accurate filling rate in the observation group was 93.33 and was significantly higher than that in the control group. The success rate of root canal operation in the observation group was 90.00g, which was significantly higher than that in the control group (66.67). [conclusion] Nickel-titanium instrument can relieve the pain and improve the clinical efficacy of root canal therapy, which may be related to the inhibition of stress response.
