安氏Ⅱ類亞類成人顳下頜關(guān)節(jié)形態(tài)和位置的CBCT研究

【摘要】:目的:安氏II類亞類患者的治療是臨床上討論和研究的熱點(diǎn),安氏II類亞類患者中有一部分存在不同程度的顳下頜關(guān)節(jié)的癥狀。對(duì)安氏II類亞類患者顳下頜關(guān)節(jié)的研究有重要的臨床意義。錐體束CT(Cone-beamcomputed tomography,CBCT)是目前用于口腔頜面部最先進(jìn)的影像學(xué)檢查和診斷手段。本研究應(yīng)用CBCT,通過(guò)比較安氏II類亞類患者和個(gè)別正常牙合且關(guān)節(jié)健康人群髁突的形態(tài)和關(guān)節(jié)間隙,研究安氏II類亞類患者顳下頜關(guān)節(jié)的形態(tài)和位置特征,從而為臨床工作提供依據(jù)和參考。 材料和方法:選擇安氏II類亞類患者20例,其中男性10例,,女性10例,年齡為22~30歲,平均年齡24.6歲;關(guān)節(jié)健康的個(gè)別正常牙合20例,其中男性10例,女性10例,年齡為19~26歲,平均年齡22.4歲。所有研究對(duì)象知情同意。用CBCT系統(tǒng)KaVo3D eXam進(jìn)行掃描和成像,系統(tǒng)掃描儀獲取影像數(shù)據(jù)后,導(dǎo)入到計(jì)算機(jī)工作站中,所得圖像以Dicom3.0醫(yī)學(xué)數(shù)字圖像通訊標(biāo)準(zhǔn)存儲(chǔ)。應(yīng)用Invivo5軟件來(lái)讀取已存儲(chǔ)的圖像數(shù)據(jù),并進(jìn)行CT圖像的三維重建。然后,應(yīng)用該軟件對(duì)重建的圖像進(jìn)行定位調(diào)整,設(shè)置三維方向上的坐標(biāo)軸,從三維方向?qū)?D圖像進(jìn)行定位,以達(dá)到測(cè)量標(biāo)準(zhǔn)的一致性。使用Invivo5軟件對(duì)顳下頜關(guān)節(jié)進(jìn)行三維重建和測(cè)量。 測(cè)量項(xiàng)目包括兩部分,分別為反映髁突形態(tài)的測(cè)量項(xiàng)目和反映髁突位置的測(cè)量項(xiàng)目。反映髁突形態(tài)的測(cè)量項(xiàng)目有髁突前后徑(anteroposteriordiameter)、髁突內(nèi)外徑(medialateral diameter)、髁突高度(height ofcondylar)、髁頂高度(height of the top of condylar,)、髁突體積(volume)、最大軸面面積(Maximum area);反映髁突位置的測(cè)量項(xiàng)目有髁突內(nèi)外徑-矢狀面角(medialateral diameter-sagittal plane angle)、髁突中心-正中矢狀面距(the center of condylar-mid sagittal plane distance)、關(guān)節(jié)內(nèi)間隙(Innerjoint space)、關(guān)節(jié)中間隙(Medial joint space)、關(guān)節(jié)外間隙(Outer jointspace)、關(guān)節(jié)上間隙(Superior joint space)、關(guān)節(jié)后間隙(Posterior jointspace)、關(guān)節(jié)前間隙(Anterior joint space)。 采用SPSS13.0統(tǒng)計(jì)學(xué)分析軟件,對(duì)各項(xiàng)數(shù)據(jù)進(jìn)行正態(tài)性檢驗(yàn),若服從正態(tài)則采用均數(shù)(X)和標(biāo)準(zhǔn)差(S)進(jìn)行描述,在Ⅱ類亞類組和正常牙合組中分別采用獨(dú)立樣本t檢驗(yàn)分析是否存在性別差異,Ⅱ類亞類組和正常牙合組內(nèi)左右兩側(cè)通過(guò)采用配對(duì)t檢驗(yàn)分析測(cè)量項(xiàng)目研究是否存在差異,采用獨(dú)立樣本t檢驗(yàn)對(duì)Ⅱ類亞類組和正常牙合組的測(cè)量項(xiàng)目進(jìn)行分析,研究?jī)山M間是否存在差異;若不服從正態(tài)分布則采用中位數(shù)(M)和四分位數(shù)間距(Q)進(jìn)行描述,采用Wilcoxon秩和檢驗(yàn)進(jìn)行統(tǒng)計(jì)分析。檢驗(yàn)標(biāo)準(zhǔn)α0.05。 結(jié)果: 1正常牙合組左右側(cè)對(duì)比中,所有測(cè)量項(xiàng)目的差異均無(wú)統(tǒng)計(jì)學(xué)意義。 2Ⅱ類亞類組男性和女性中性側(cè)與遠(yuǎn)中側(cè)差值的對(duì)比,所有測(cè)量項(xiàng)目P值均大于0.05。即Ⅱ類亞類組雙側(cè)關(guān)節(jié)測(cè)量數(shù)據(jù)差值的性別差異沒(méi)有統(tǒng)計(jì)學(xué)意義。 3Ⅱ類亞類組中性側(cè)和遠(yuǎn)中側(cè)的比較中,遠(yuǎn)中側(cè)小于中性側(cè)的測(cè)量項(xiàng)目有:髁突前后徑中性側(cè)11.48±2.25mm,遠(yuǎn)中側(cè)9.15±1.98mm、內(nèi)外徑中性側(cè)19.49±1.82mm,遠(yuǎn)中側(cè)17.57±2.50mm、髁突高度中性側(cè)20.74±3.42mm,遠(yuǎn)中側(cè)19.00±3.33mm、髁頂高度中性側(cè)9.93±2.23mm,遠(yuǎn)中側(cè)8.15±1.81mm、髁突體積中性側(cè)1048.80±322.76mm3,遠(yuǎn)中側(cè)838.45±277.30mm3、最大軸面面積中性側(cè)122.21±30.47mm2,遠(yuǎn)中側(cè)106.36±27.35mm2、關(guān)節(jié)中間隙中性側(cè)2.92±0.61mm,遠(yuǎn)中側(cè)2.82±0.59mm、外間隙中性側(cè)3.02±0.97mm,遠(yuǎn)中側(cè)2.48±1.02mm、上間隙中性側(cè)3.28±0.75mm,遠(yuǎn)中側(cè)2.71±0.68mm,后間隙中性側(cè)2.68±0.75mm,遠(yuǎn)中側(cè)2.57±0.81mm。遠(yuǎn)中側(cè)大于中性側(cè)的測(cè)量項(xiàng)目是內(nèi)外徑-矢狀面角,中性側(cè)70.77±5.79°,遠(yuǎn)中側(cè)74.87±4.37°。 4正常牙合組左右側(cè)差值與Ⅱ類亞類組中性側(cè)和遠(yuǎn)中側(cè)差值的對(duì)比中,Ⅱ類亞類組大于正常牙合組的測(cè)量項(xiàng)目有:髁突前后徑-0.68±0.88、2.39±1.67mm,髁突內(nèi)外徑-0.12±0.47mm、0.92±1.84mm,髁突高度0.08±1.10mm、1.78±1.46mm,髁頂高度-0.09±0.56mm、-3.43±3.36mm,髁突體積-0.40±41.13mm、210.35±154.85mm,最大軸面面積1.31±5.82mm、15.84±12.57mm,內(nèi)外徑-矢狀面角0.68±2.01°、-4.11±3.27°,關(guān)節(jié)外間隙0.03±0.13mm、0.55±0.42mm,關(guān)節(jié)上間隙0.02±0.14mm、0.57±0.43mm。 結(jié)論: 1Ⅱ類亞類患者遠(yuǎn)中側(cè)髁突比中性側(cè)小。 2遠(yuǎn)中側(cè)髁突位置基本正常,中性側(cè)髁突在關(guān)節(jié)窩中位置偏向前、下、內(nèi)。
[Abstract]:Objective : To study the clinical significance and clinical significance of the treatment of patients with Class II sub - type . Cone - beam computed tomography ( CBCT ) is one of the most advanced imaging and diagnostic methods used in oral and maxillofacial region . Cone - beam computed tomography ( CBCT ) is the most advanced imaging and diagnostic method used in oral and maxillofacial region . Materials and Methods : 20 male patients with type II were selected , 10 of them were male and 10 were female . The age ranged from 22 to 30 years . The mean age was 24 . 6 years .
There were 20 cases of joint health , including 10 male , 10 female , age 19 - 26 years , average age of 22.4 years . All the subjects were informed consent . After acquiring image data by CBCT system KaVo3D eXam , the acquired image was stored in the computer workstation . The acquired image was stored in Dicom3.0 medical digital image communication standard . Then , the software was applied to locate the reconstructed image , and the 3D image was positioned from the three - dimensional direction to achieve the consistency of the measurement standard .

The measurement items include two parts : the measurement items reflecting the condyle morphology and the measurement items that reflect the condylar process . The measurement items that reflect the condyle form include the condyle ' s diameter , the medial condyle diameter , the height of condyle , the height of the top of condyle , the condyle volume and the maximum area ;
The measurement items that reflect the condyle location have medial and external sagittal plane angles , the center of medial sagittal plane distance , the center of medial sagittal plane distance , the medial joint space , the medial joint space , the outer jointspace , superior joint space , posterior joint space , anterior joint space .

SPSS 13.0 was used to analyze the normality of the data , and the mean ( X ) and standard deviation ( S ) were used to describe whether there were gender differences in the group 鈪
