【摘要】：目的研究在種植上頜竇提升過程中,造成手術(shù)出血的上頜竇骨內(nèi)血管在中國人群中的分布情況,分析血管的存在對手術(shù)的影響。方法納入年齡35歲以上,拍攝CBCT包含雙側(cè)清晰上頜竇影像的患者進行分析。共納入患者150例(300側(cè)上頜竇)。觀察相應區(qū)域的骨內(nèi)血管是否存在,測量血管離竇底的垂直距離,以及血管的直徑。研究由2名實驗員獨立進行觀察和測量,并分析數(shù)據(jù)。結(jié)果共發(fā)現(xiàn)有122側(cè)上頜竇區(qū)域有骨性血管存在,占患者總數(shù)的40.6%。血管距離竇底的平均距離為(7.19±1.36)mm,而血管的平均直徑為(1.32±0.51)mm。在發(fā)現(xiàn)率上,男性較女性高,血管的高度上男性高于女性,血管直徑上男性粗于女性,差異均無統(tǒng)計學意義。雙側(cè)血管的存在和形態(tài)上具有一定對稱性。結(jié)論上頜竇區(qū)域骨內(nèi)血管的粗大和較低的垂直高度可能會造成種植外提升手術(shù)中的出血,術(shù)前應拍攝CBCT確定在手術(shù)區(qū)域有無粗大血管影響,但陰性的結(jié)果并不能排除損傷血管的可能性。手術(shù)時應該控制開窗范圍避免并發(fā)癥的產(chǎn)生。
[Abstract]:Objective to study the distribution of intraosseous vessels of maxillary sinus caused by surgical bleeding in Chinese population during the implantation of maxillary sinus, and to analyze the influence of the presence of blood vessels on the operation. Methods patients over 35 years old who were photographed with bilateral clear maxillary sinus images on CBCT were analyzed. A total of 150 patients with 300 sides of maxillary sinus were included. The presence of intraosseous vessels, the vertical distance from the sinus floor and the diameter of the blood vessels were observed. The study was observed and measured independently by two experimenters, and the data were analyzed. Results there were 122 osseous vessels in the maxillary sinus area, accounting for 40.6% of the total number of patients. The average distance from the bottom of the sinus was 7.19 鹵1.36 mm, and the average diameter of the blood vessel was 1.32 鹵0.51mm. The detection rate was higher in male than in female, the height of blood vessel in male was higher than that in female, and the diameter of vessel in male was thicker than that in female, the difference was not statistically significant. The existence and morphology of bilateral blood vessels have certain symmetry. Conclusion the large and low vertical height of the intraosseous vessels in the maxillary sinus region may result in hemorrhage in the extracellular lifting operation. Before operation, CBCT should be taken to determine if there is any influence of the large blood vessels in the area of operation. But negative results do not rule out the possibility of blood vessel damage. The scope of window opening should be controlled during surgery to avoid complications.
