【摘要】：目的探討下頜骨骨折行堅固內(nèi)固定術(shù)后感染相關(guān)的風(fēng)險因素,為進一步完善下頜骨骨折患者圍手術(shù)期的治療管理提供依據(jù),降低下頜骨骨折行堅固內(nèi)固定術(shù)后感染發(fā)生率。方法設(shè)計病例對照研究并收集我院2006年1月-2012年12月期間下頜骨骨折堅固內(nèi)固定手術(shù)患者447例,其中出現(xiàn)術(shù)后感染的病例為26例(實驗組)。從每例發(fā)生術(shù)后感染的患者入院前2周內(nèi),隨機選取1例下頜骨骨折堅強內(nèi)固定術(shù)后無任何并發(fā)癥的患者納入對照組,共26例。收集整理相關(guān)的變量數(shù)據(jù),采用SAS9.2軟件包在2組間行二變量和多因素Logistic回歸分析,確定與術(shù)后感染相關(guān)的因素。結(jié)果回歸分析結(jié)果顯示,增長的下頜骨骨折嚴重程度評分(OR=2.509,95%CI為1.083-5.814)以及患者欠佳的全身健康狀態(tài)(OR=5.019,95%CI為1.294-19.472)是導(dǎo)致術(shù)后感染的相關(guān)風(fēng)險因素。在二變量檢驗中抗生素使用天數(shù)與術(shù)后感染具有相關(guān)性(P0.05)。結(jié)論骨折的嚴重程度和患者欠佳的身體健康狀態(tài)是下頜骨骨折術(shù)后感染的風(fēng)險因素。
[Abstract]:Objective to explore the risk factors related to infection after rigid internal fixation of mandibular fracture, to provide basis for further improvement of perioperative management of mandibular fracture and to reduce the incidence of infection after rigid internal fixation of mandibular fracture. Methods A case-control study was conducted in 447 patients with rigid internal fixation of mandibular fractures from January 2006 to December 2012 in our hospital. Among them 26 cases were infected after operation (experimental group). Within 2 weeks before admission, one patient with no complications after rigid internal fixation of mandibular fracture was randomly selected as control group (n = 26). The data of related variables were collected and analyzed. The two variables and multivariate Logistic regression analysis were carried out between the two groups by SAS9.2 software package to determine the factors related to postoperative infection. Results the regression analysis showed that the increased mandibular fracture severity score (OR 2.509 95 CI = 1.083-5.814) and the patients' poor general health state (CI = 1.294-19.472) were the risk factors associated with postoperative infection. There was a correlation between the days of antibiotic use and postoperative infection in two-variable test (P 0. 05). Conclusion the severity of fracture and poor health of patients are risk factors for postoperative infection of mandibular fracture.
