牙周炎犬即刻種植即刻加載后種植體—骨界面骨結(jié)合的研究

【摘要】:目的研究犬下前磨牙區(qū)域建立牙周炎模型后即刻種植即刻加載種植體-骨界面骨結(jié)合情況。 方法8只實(shí)驗(yàn)用Beagle犬,所有犬于雙側(cè)下頜前磨牙位置行絲圈結(jié)扎并輔以高糖飲食,建立犬牙周炎模型。開始建模后的第1、2、3、4周分別對所有犬行牙周探診,建模在4周時(shí)間內(nèi)完成。第5周分別給予所有犬齦上潔治及齦下刮治并控制牙周菌斑。5周后同時(shí)拔除每只犬雙側(cè)下頜第3、4前磨牙,在相應(yīng)的拔牙窩選擇最佳位置即刻植入DIO種植體(直徑3.8mm,長度10.0mm)各1顆,同期連接基臺(直徑3.8mm,長度6.0mm)。術(shù)后24h內(nèi)采用種植體垂直加載儀進(jìn)行垂直加載,將每只犬下頜的4枚種植體按照加載力的不同分為:5N、10N、15N、對照組,加載頻率為2Hz,加載時(shí)間為每日15min。加載4周及8周后各處死4只犬,制作標(biāo)本,分別進(jìn)行大體觀察、影像學(xué)觀察與分析、硬組織與形態(tài)學(xué)觀察分析。 結(jié)果①各組種植體周圍骨質(zhì)均有吸收,其中15N加載組吸收明顯并伴有Ⅱ-Ⅲ度松動。②不同加載周期,5、10N加載組較對照組在BMD方面差異有統(tǒng)計(jì)學(xué)意義(P0.01)。③不同加載周期,5、10N加載組較對照組在BV/TV、Tb.Th、Tb.N、Po方面差異均有統(tǒng)計(jì)學(xué)意義(P0.01),加載8周時(shí),5、10N加載組與對照組在SMI方面差異有統(tǒng)計(jì)學(xué)意義(P0.01)。④組織切片顯示不同加載期5、10N加載組較對照組新骨骨量更多,加載8周時(shí)5、10N加載組較對照組新生骨與螺紋間空隙更小。 結(jié)論在本實(shí)驗(yàn)條件下,不同加載周期時(shí),加載組較對照組骨生成及骨改建更積極,表明牙周炎模型犬即刻種植后,以適當(dāng)?shù)募虞d力即刻加載后可獲得良好的種植體-骨結(jié)合。
[Abstract]:Objective to study the bone bonding between implant-bone interface and immediate implant implantation after periodontitis model was established in canine premolar region.Methods eight Beagle dogs were used to establish a canine model of periodontitis.Periodontal diagnoses were performed in all dogs in 4 weeks after modeling, and the modeling was completed within 4 weeks.All dogs were given supragingival scaling and subgingival curettage at the 5th week, and the periodontal plaque was controlled for 5 weeks.DIO implants (3. 8mm in diameter and 10. 0mm in length) were immediately implanted in the optimal position of the extraction fossa. The abutment was connected with the abutment (3. 8mm in diameter and 6. 0mm in length) at the same time.Four mandibular implants of each dog were divided into two groups according to the different loading forces: 1: 5NX 10Nm 15N, the control group was loaded with 2Hz, and the loading time was 15min per day.After 4 weeks and 8 weeks of loading, 4 dogs were killed, and the specimens were made for gross observation, imaging observation and analysis, hard tissue and morphology observation and analysis.Results 1 the bone around implants were absorbed in each group.The absorption of 15N loading group was significantly higher than that of the control group with 鈪,
