【摘要】：目的：對顳下頜關(guān)節(jié)盤前移位患者用穩(wěn)定型咬合板進(jìn)行可逆性咬合治療,觀察及探討穩(wěn)定型咬合板對顳下頜關(guān)節(jié)盤前移位的療效,從而對顳下頜關(guān)節(jié)盤前移位的臨床治療起到指導(dǎo)意義。方法：按照病例納入排除標(biāo)準(zhǔn),選取42例來我院顳下頜關(guān)節(jié)�？崎T診就診的顳下頜關(guān)節(jié)盤前移位患者(可復(fù)性或不可復(fù)性,主要癥狀為關(guān)節(jié)彈響、關(guān)節(jié)區(qū)疼痛、不同程度的張口受限等),治療前均進(jìn)行臨床診斷及MRI等影像學(xué)檢查,并對所有患者用影像學(xué)方法排除關(guān)節(jié)的器質(zhì)性病變,然后均用上頜穩(wěn)定型咬合板治療。觀察治療前及治療后2、4、6周及3個(gè)月患者張口度、關(guān)節(jié)彈響及疼痛分級指數(shù)(visual analogue scale, VAS)和helkimo指數(shù)變化。結(jié)果：在應(yīng)用穩(wěn)定型咬合板治療的42例患者中,所有患者均療效顯著,與治療前比較,穩(wěn)定型咬合板治療后2、4、6周及3個(gè)月VAS值、helkimo指數(shù)均逐漸降低,而張口度逐漸增大,其差異有統(tǒng)計(jì)學(xué)意義(P0.05)。20例有關(guān)節(jié)彈響者戴用穩(wěn)定型咬合板后,彈響消失13例、減輕4例、無變化3例,有效13/20。臨床上收到較滿意的效果。結(jié)論：用穩(wěn)定型咬合板治療顳下頜關(guān)節(jié)盤前移位能明顯減輕疼痛及關(guān)節(jié)區(qū)彈響,改善張口度,是治療顳下頜關(guān)節(jié)盤前移位的一種有效手段。
[Abstract]:Objective: to evaluate the effect of stable occlusal plate on anterior disc displacement of temporomandibular joint (TMJ). Therefore, it is of guiding significance for the clinical treatment of temporomandibular joint anterior disc displacement. Methods: according to the exclusion criteria, Forty-two patients with anterior disc displacement of temporomandibular joint (temporomandibular joint) were selected. Before treatment, clinical diagnosis and MRI examination were performed in all patients, and all patients were excluded from the organic diseases of the joint by imaging method. Then all the patients were treated with maxillary stable occlusal plate. Results: among the 42 patients treated with stable occlusal plate, all of them had significant curative effects, compared with those before treatment, the changes of visual analogue scale (vas) and helkimo index were significant in all of the 42 patients treated with stable occlusal plate. The VAS index of stable occlusal plate decreased gradually at 6 weeks and 3 months after treatment, and the degree of mouth opening gradually increased. The difference was statistically significant (P 0.05). 20 cases with articular clasps wearing stable occlusal plate, 13 cases disappeared and 4 cases decreased. No change was found in 3 cases, and the effective rate was 13 / 20. Conclusion: the treatment of anterior disc displacement of temporomandibular joint with stable occlusal plate can significantly reduce the pain and the bounce of articular area, and improve the degree of mouth opening. It is an effective method to treat anterior disc displacement of temporomandibular joint.
