基底冠材料和形態(tài)設(shè)計(jì)對種植固定義齒被動適合性影響的研究

【摘要】:目的1.研究氧化鋯全瓷和純鈦烤瓷兩種修復(fù)材料對計(jì)算機(jī)輔助設(shè)計(jì)及制作的種植體支持粘結(jié)固位義齒被動適合性的影響。2.研究360°頸環(huán)設(shè)計(jì)對計(jì)算機(jī)輔助設(shè)計(jì)及制作的種植固定義齒被動適合性的影響。3.探究飾瓷燒結(jié)前后不同基底冠的修復(fù)體被動適合性的變化情況。4.通過檢測種植體周圍樹脂的應(yīng)變,探究種植固定義齒周圍應(yīng)力分布的情況。方法調(diào)配環(huán)氧樹脂使之彈性模量與牙槽骨相近,制作成32個(gè)大小相同的樹脂塊并在每塊植入兩枚種植體,模擬左下頜第二前磨牙至第二磨牙缺失后的種植修復(fù)。在每個(gè)種植體頸部頰側(cè)、舌側(cè)、近中、遠(yuǎn)中四個(gè)方向上分別粘結(jié)應(yīng)力感應(yīng)器。采用計(jì)算機(jī)輔助設(shè)計(jì)及制作4組不同材料及形態(tài)設(shè)計(jì)的三單位固定義齒內(nèi)冠,每組8個(gè)(n=8)。A組為常規(guī)氧化鋯全瓷冠組,B組為360°氧化鋯頸環(huán)基底全瓷冠組,C組為常規(guī)純鈦烤瓷冠組,D組為360°純鈦金屬頸環(huán)基底烤瓷冠組。利用應(yīng)力感應(yīng)裝置分別測量內(nèi)冠和經(jīng)燒結(jié)飾瓷的最終修復(fù)體粘結(jié)于種植體后,各組種植體周圍應(yīng)變。用SPSS分析軟件進(jìn)行數(shù)據(jù)處理。結(jié)果1.A、B、C、D四組各部位應(yīng)力感應(yīng)器均感應(yīng)到應(yīng)力。2.飾瓷燒結(jié)前,不同的內(nèi)冠材料對應(yīng)變有影響,氧化鋯組應(yīng)變較小(P0.01);不同內(nèi)冠形態(tài)對應(yīng)變有影響,有頸環(huán)組應(yīng)變較小(P0.01);材料與形態(tài)存在交互作用(P0.01)。3.飾瓷燒結(jié)后,不同的內(nèi)冠材料對最終修復(fù)體所產(chǎn)生的應(yīng)變有影響,氧化鋯組應(yīng)變較小(P0.01);不同內(nèi)冠形態(tài)對最終修復(fù)體所產(chǎn)生的應(yīng)變有影響,有頸環(huán)組應(yīng)變較小(P0.01);材料與形態(tài)存在交互作用,氧化鋯有頸環(huán)組應(yīng)變最小(P0.01)。4.經(jīng)飾瓷燒結(jié),各組應(yīng)變均增加。不同材料內(nèi)冠對烤瓷前后應(yīng)變差異無影響(P0.05);不同內(nèi)冠形態(tài)對烤瓷前后應(yīng)變差異有影響,有360°頸環(huán)組差異較小(P0.01)。5.A、B、C、D四組經(jīng)飾瓷燒結(jié)后,近遠(yuǎn)中方向上的應(yīng)變大于其頰舌方向上的應(yīng)變,近負(fù)荷點(diǎn)處應(yīng)變最大,有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論1.CAD/CAM制作的種植體支持的固定義齒無法達(dá)到絕佳的被動適合性。2.氧化鋯全瓷比純鈦金屬烤瓷修復(fù)體具有更好的被動適合性。3.360°頸環(huán)有利于提高種植固定義齒被動適合性。4.氧化鋯全瓷固定義齒及純鈦金屬烤瓷固定義齒經(jīng)飾瓷燒結(jié)后,其被動適合性均降低;360°頸環(huán)有利于減輕由燒結(jié)飾瓷帶來的不良影響,而材料對烤瓷導(dǎo)致的適合性變化無影響。5.臨床上應(yīng)采取措施減小種植固定義齒頸部周圍近遠(yuǎn)中方向,尤其是兩種植體近橋體側(cè)位置的應(yīng)力。
[Abstract]:Objective 1. To study the effect of zirconia all-ceramic and pure titanium porcelain on the passive suitability of implant supported adhesive retention denture. 2. To study the influence of 360 擄neck ring design on computer aided design and fabrication. Study on the change of passive fitness of restoration with different base crowns before and after sintering. 4. By detecting the strain of resin around the implant, To investigate the stress distribution around the implant fixed denture. Methods Epoxy resin was blended to make the elastic modulus close to the alveolar bone, and 32 resin blocks of the same size were made and two implants were implanted in each implant. Imitating implant restoration after left mandibular second premolar to second molar missing. In each implant neck, buccal side, tongue side, proximal center, The four directions in the far distance are respectively bonded to the stress sensors. Four groups of three unit fixed denture crowns with different materials and shapes are designed and fabricated by means of computer aided design. Each group consisted of 8 Zirconia ceramic crowns, group B, 360 擄zirconia neck ring, all-ceramic crowns, group C, conventional pure titanium fused porcelain crowns, group D, 360 擄pure titanium metal neck ring, porcelain crowns, respectively, were measured by stress sensing apparatus. The internal crown and the final restoration of sintered porcelain are bonded to the implant, The data were processed by SPSS software. Results 1. The stress sensors in each of the four groups were sensitive to stress. 2. Before sintering, different crown materials affected the strain. The strain of zirconia group is smaller than that of zirconia group, the strain of different inner crown has influence on the strain, and the strain of neck ring group is smaller than that of P0.01.The interaction between material and morphology is P0.010.3.After sintered porcelain, different inner crown materials have an effect on the strain produced by the final restoration. The strain of Zirconia group is smaller than that of Zirconia group, the shape of different inner crown has influence on the strain produced by the final prosthesis, and the strain is smaller in the group of neck ring. There is interaction between the material and the form, and the minimum strain of Zirconia with cervical ring group is P0.01U. 4. Sintered by ceramic ornament, The strain of each group increased. The internal crown of different materials had no effect on the strain difference before and after the porcelain, and the different shape of the inner crown had an effect on the strain difference before and after the porcelain, there was a little difference in 360 擄neck ring group (P 0.01). 5. After sintering, the four groups were sintered with porcelain. The strain up near the far side is larger than the strain in the direction of the cheek and tongue, and the strain at the near load point is the largest. Conclusion the 1.CAD/CAM implant-supported fixed denture can not achieve excellent passive fitness. 2.Zirconia all-ceramic has better passive fitness than pure titanium metal ceramic prosthesis. 3.360 擄neck ring is good for improving the species. Zirconia all porcelain fixed denture and pure titanium metal porcelain fixed denture after sintered with porcelain, All the passive adaptations were reduced by 360 擄neck ring, and the adverse effects caused by sintered porcelain were alleviated, while the material had no effect on the change of fitness caused by porcelain. 5. In clinic, measures should be taken to reduce the near and far side direction around the neck of implant fixed denture. Especially, the stress of the two implants near the side of the bridge.
