【摘要】：研究背景:口腔扁平苔蘚是一種復(fù)層鱗狀上皮細(xì)胞慢性炎癥性疾病,發(fā)生于黏膜和皮膚,可單獨(dú)或同時(shí)發(fā)病。由于口腔扁平苔蘚較正常組織具有更高的惡變傾向,WHO已經(jīng)將其定義為癌前狀態(tài),其癌變率在0.4%-2%�？谇击[狀細(xì)胞癌是最常見(jiàn)的口腔頜面部惡性腫瘤,約占口腔所有腫瘤的90%,5年生存率只有35%-50%�？谇击[狀細(xì)胞癌具有惡性程度高、局部浸潤(rùn)性強(qiáng)、早期淋巴結(jié)轉(zhuǎn)移、預(yù)后差的特征。P53是一種至關(guān)重要的抑癌基因,在食道癌、乳腺癌、膀胱癌、肝癌及宮頸癌等人類約50%的癌癥中均發(fā)現(xiàn)了p53基因的突變。在凋亡抑制蛋白家族成員中,survivin相對(duì)分子質(zhì)量最小、凋亡抑制作用最強(qiáng),它與增殖、血管形成、口腔致癌作用以及口腔鱗狀細(xì)胞癌相關(guān),可能作為癌癥標(biāo)記物。目的:通過(guò)檢測(cè)p53蛋白和survivin蛋白在口腔扁平苔蘚及口腔鱗狀細(xì)胞癌中的表達(dá)情況,探討兩者之間的關(guān)系以及在口腔扁平苔蘚發(fā)生、發(fā)展、癌變之中的作用。方法:本實(shí)驗(yàn)采用免疫組織化學(xué)法檢測(cè)p53蛋白、survivin蛋白在12例正常口腔黏膜組織、30例口腔扁平苔蘚組織、30例口腔鱗狀細(xì)胞癌組織中的表達(dá)情況,實(shí)驗(yàn)標(biāo)本來(lái)自患者活檢或手術(shù)標(biāo)本取得組織。應(yīng)用SPSS軟件分析實(shí)驗(yàn)結(jié)果。結(jié)果:1.P53蛋白在正�？谇火つそM織、口腔扁平苔蘚組織和口腔鱗狀細(xì)胞癌組織中的陽(yáng)性表達(dá)依次增高,陽(yáng)性表達(dá)率分別為16.67%(2/12)、53.33%(16/30)、80.00%(24/30),組間差別有統(tǒng)計(jì)學(xué)意義(p0.05)。2.Survivin蛋白在正常口腔黏膜組織、口腔扁平苔蘚組織和口腔鱗狀細(xì)胞癌組織中的陽(yáng)性表達(dá)依次增高,陽(yáng)性表達(dá)率分別為0%(0/12)、26.67%(8/30)、56.67%(17/30),組間差別有統(tǒng)計(jì)學(xué)意義(p0.05)。3.P53蛋白和survivin蛋白表達(dá)呈顯著正相關(guān)(p0.01)。結(jié)論:1.P53蛋白在正常口腔黏膜組織、口腔扁平苔蘚組織和口腔鱗狀細(xì)胞癌組織中的陽(yáng)性表達(dá)率依次升高,提示p53蛋白在從口腔扁平苔蘚到口腔鱗狀細(xì)胞癌的發(fā)展中起促進(jìn)作用。2.Survivin蛋白在正�？谇火つそM織、口腔扁平苔蘚組織和口腔鱗狀細(xì)胞癌組織中的陽(yáng)性表達(dá)率依次升高,提示survivin蛋白在從口腔扁平苔蘚到口腔鱗狀細(xì)胞癌的發(fā)展中起促進(jìn)作用。3.P53蛋白和survivin蛋白在口腔扁平苔蘚癌變過(guò)程中的表達(dá)呈正相關(guān),提示兩者在從口腔扁平苔蘚到口腔鱗狀細(xì)胞癌的過(guò)程中可能起協(xié)同作用。
[Abstract]:Background: oral lichen planus (OLP) is a chronic inflammatory disease of squamous epithelium, which occurs in mucosa and skin. Oral lichen planus has a higher tendency to malignant change than normal tissue. The WHO has defined it as precancerous state, and its canceration rate is 0.4-2. Oral squamous cell carcinoma is the most common malignant tumor in oral and maxillofacial region. About 90% of all oral tumors, the 5-year survival rate is only 35% -50%. Oral squamous cell carcinoma has the characteristics of high malignancy, strong local invasion, early lymph node metastasis and poor prognosis. P53 is a crucial suppressor gene in esophageal cancer and breast cancer. P53 gene mutations were found in about 50% of human cancers such as bladder cancer, liver cancer and cervical cancer. Survivin has the least molecular weight and the strongest inhibitory effect on apoptosis in the family of apoptosis suppressor proteins, which is associated with proliferation and angiogenesis. Objective: to detect the expression of p53 protein and survivin protein in oral lichen planus and oral squamous cell carcinoma. To explore the relationship between the two and the occurrence and development of oral lichen planus, Methods: immunohistochemical method was used to detect the expression of p53 protein survivin in 30 cases of oral lichen planus and 30 cases of oral squamous cell carcinoma (OSCC). The results were analyzed by SPSS software. Results the positive expression of 1. 1. P53 protein was increased in normal oral mucosa, oral lichen planus and oral squamous cell carcinoma. The positive expression rates of survivin protein in normal oral mucosa, oral lichen planus and oral squamous cell carcinoma were significantly higher than those in normal oral mucosa, oral lichen planus and oral squamous cell carcinoma. The positive expression rates were 0 / 12 / 26.67 and 56. 67, respectively. There was a significant positive correlation between the expression of p53 protein and survivin protein in normal oral mucosa. Conclusion: 1. P53 protein is expressed in normal oral mucosa, and there is a significant positive correlation between the expression of p53 protein and survivin protein in normal oral mucosa. The positive rate of p53 protein in oral lichen planus and oral squamous cell carcinoma increased in turn, suggesting that p53 protein plays a role in promoting the development from oral lichen planus to oral squamous cell carcinoma. 2. Survivin protein in normal oral mucosa. The positive expression rates of oral lichen planus and oral squamous cell carcinoma increased in turn. The results suggest that the expression of survivin protein and survivin protein in oral lichen planus carcinogenesis is positively correlated with the development of oral lichen planus. These results suggest that both may play a synergistic role in the process from oral lichen planus to oral squamous cell carcinoma.
