鈦板重建下頜骨體部缺損的生物力學(xué)變化

【摘要】:背景:三維有限元在口腔領(lǐng)域應(yīng)用廣泛,但關(guān)于下頜骨體部缺損鈦板重建的三維有限元研究不多。目的:三維有限元分析鈦板重建下頜骨體部缺損的生物力學(xué)特點(diǎn)。方法:建立下頜骨體部缺損雙皮質(zhì)鈦釘鈦板重建的三維有限元模型,模擬正常咬合狀態(tài),在右側(cè)下頜骨第一磨牙咬合面中央窩垂直加載200 N的載荷,分析下頜骨、鈦釘、鈦板的應(yīng)力分布和最大位移。結(jié)果與結(jié)論:(1)模擬咀嚼運(yùn)動加載后下頜骨的應(yīng)力在下頜體和下頜支部比較集中,其中應(yīng)力集中比較明顯的部位是下頜支的前后緣和下頜骨的下緣,下頜骨后緣的應(yīng)力小于下頜骨前緣的應(yīng)力,鈦板固定和骨質(zhì)接觸部位也出現(xiàn)應(yīng)力集中;(2)雙皮質(zhì)鈦釘近螺帽處應(yīng)力值最大,鈦釘和鈦板接觸部位出現(xiàn)應(yīng)力集中現(xiàn)象;(3)下頜骨升支部鈦釘?shù)淖畲髴?yīng)力值大于缺損前端鈦釘最大應(yīng)力值;(4)鈦板的應(yīng)力集中部位主要為鈦釘固定部位,鈦板上緣和下緣的應(yīng)力峰值主要集中在下頜骨缺損前端和鈦釘接觸區(qū)以及下頜骨升支的鈦釘接觸區(qū);(5)鈦板重建下頜骨體部缺損后位移比較容易發(fā)生在缺損前端下頜骨和鈦板接觸部位;(6)結(jié)果表明,下頜骨體部缺損雙皮質(zhì)鈦釘鈦板固定重建相對比較穩(wěn)定,下頜角前端的鈦板比較容易發(fā)生斷裂。
[Abstract]:Background: 3D finite element is widely used in the field of stomatology. But there are few three-dimensional finite element studies on the reconstruction of mandibular body defect titanium plate. Objective: to analyze the biomechanical characteristics of mandibular body defect reconstruction with titanium plate. Three-dimensional finite element model of titanium plate reconstruction, In order to simulate the normal occlusal state, the first molar of the right mandible was subjected to 200 N vertical loading in the central fossa of the first molar, and the mandible and titanium nail were analyzed. Results and conclusion\\\ the stress of mandible after masticatory movement was concentrated in mandibular body and mandibular branch, and the stress concentration was in the anterior and posterior margin of mandibular branch and lower margin of mandible. The stress of the posterior edge of the mandible was smaller than that of the anterior edge of the mandible, and the stress concentration was also found in the fixation of titanium plate and the contact site of the bone. Stress concentration in the contact site of titanium nail and titanium plate; (3) the maximum stress value of titanium nail in the ascending branch of mandible is greater than the maximum stress value of titanium nail in front end of the defect; (4) the stress concentration position of titanium plate is mainly fixed by titanium nail. The peak stress of the upper and lower edge of titanium plate is mainly concentrated in the front end of mandibular defect and contact area of titanium nail, and the contact area of titanium nail of ascending branch of mandible.) the displacement after reconstruction of mandibular body defect with titanium plate is more likely to occur at the front end of the defect. The results of maxillary contact with titanium plate showed that, The reconstruction of the mandibular body defect was relatively stable, and the titanium plate at the anterior end of the mandibular angle was prone to fracture.
