類金剛石鍍膜可調(diào)一體式磁力擴弓矯治器矯治效果的動物實驗研究

【摘要】:實驗?zāi)康? 通過動物實驗,借助錐束計算機體層掃描技術(shù)(CBCT)和組織學(xué)觀察,對類金剛石鍍膜的可調(diào)一體式磁力擴弓矯治器與傳統(tǒng)螺旋擴弓矯治器的擴弓效果進(jìn)行對比,為進(jìn)一步對可調(diào)一體式磁力擴弓矯治器的基礎(chǔ)研究和臨床實踐打下基礎(chǔ)。 實驗方法: 1、可調(diào)一體式磁力擴弓矯治器力學(xué)性能測定 選擇N52型高磁能積釹鐵硼磁體制作可調(diào)一體式磁力擴弓矯治器,磁體規(guī)格為:11.5mm×2.73mm×2.3mm。使用INSTRON萬能測力計測定磁力擴弓矯治器作用范圍為3mm的兩次加力的力-距曲線。 2、動物實驗研究 選擇6月齡,恒牙列初期,無明顯頜牙合缺陷,體重約8公斤的健康雄性比格犬18只,隨機分為3組:磁力擴弓器組6只、螺旋擴弓器組6只、對照組6只。在所有實驗犬上頜雙側(cè)第一前磨牙(PM1)、第四前磨牙(PM4)頰尖連線上,腭中縫兩側(cè)旁開約3mm處垂直骨面分別旋入自攻自斷牙本質(zhì)釘作為定位標(biāo)記,制取磁力擴弓器組和螺旋擴弓器組實驗犬上頜模型,,根據(jù)模型分別制作兩組樣本的擴弓矯治器。磁力擴弓器組戴用可調(diào)一體式磁力擴弓矯治器,螺旋擴弓器組戴用傳統(tǒng)螺旋擴弓矯治器,擴弓器放置于雙側(cè)尖牙牙尖與第一磨牙近中頰尖連線形成的四邊形的對角線的交點處,對照組不戴用擴弓矯治器。螺旋擴弓器組每周加力兩次,每次旋轉(zhuǎn)3/4圈,磁力擴弓器組于實驗兩周時通過機械限位裝置,將磁體間距調(diào)回到0加力一次,4周后結(jié)束擴弓,處死三組所有實驗犬。對三組所有樣本實驗犬分別在戴用擴弓矯治器前(T1)與結(jié)束擴弓后(T2)進(jìn)行全頭顱CBCT掃描記錄并分別測量T1、T2期上頜尖牙間寬度、第四前磨牙頰尖間寬度、第一磨牙近中頰尖間寬度、第四前磨牙傾斜度、第一磨牙(M1)傾斜度、第一前磨牙區(qū)定位釘和第四前磨牙區(qū)定位釘間距,用SPSS統(tǒng)計軟件進(jìn)行數(shù)據(jù)統(tǒng)計和分析。三組所有實驗犬處死后,截取兩側(cè)第一前磨牙間、擴弓器中央軸鄰近處和兩側(cè)第一恒磨牙間的腭中縫軟硬組織,進(jìn)行HE染色,光學(xué)顯微鏡下觀察腭中縫組織形態(tài)學(xué)改變。 實驗結(jié)果: 1、可調(diào)一體式磁力擴弓矯治器力學(xué)性能 磁力擴弓矯治器兩次加力后,出現(xiàn)相似的力-距曲線,力值范圍為10.38N-1.63N,磁體間距為0-3mm。 2、動物實驗結(jié)果 2.1CBCT測量結(jié)果:磁力擴弓器組與螺旋擴弓器組各線距測量項目及PM4、M1傾斜度實驗后均增加,且差異有顯著性,對照組各測量項目實驗后除PM4傾斜度外亦均少量增加,且差異有顯著性。三組間所有測量項目T2-T1期的差值兩兩比較結(jié)果顯示:磁力擴弓器組與螺旋擴弓器組所有項目差值的均數(shù)均大于對照組,且差異有顯著性;磁力擴弓器組尖牙間寬度的增加量比螺旋擴弓器組大,有統(tǒng)計學(xué)差異;而螺旋擴弓器組PM1、PM4區(qū)定位釘間距的增加量比磁力擴弓器組大,亦有統(tǒng)計學(xué)差異。 2.2腭中縫區(qū)組織學(xué)改變:磁力擴弓器組與螺旋擴弓器組腭中縫均明顯增寬,骨縫中成纖維細(xì)胞、成骨細(xì)胞大量增殖,呈分泌旺盛狀態(tài),骨縫邊緣有明顯的新骨沉積,尤其是在磁力擴弓矯治器鄰近的組織區(qū)域:成骨細(xì)胞大量增殖,分泌活躍,呈星網(wǎng)細(xì)胞狀,破骨細(xì)胞明顯增多,可見大量血管斷面,其組織改建與修復(fù)程度高于螺旋擴弓器組。 結(jié)論: 1、本研究中的可調(diào)一體式磁力擴弓矯治器擴弓效果顯著 2、可調(diào)一體式磁力擴弓矯治器擴弓的骨性效應(yīng)小于螺旋擴弓矯治器,牙性效應(yīng)大于螺旋擴弓矯治器 3、采用可調(diào)一體式磁力擴弓矯治器與螺旋擴弓矯治器擴弓均能明顯刺激腭中縫進(jìn)行骨改建 4、可調(diào)一體式磁力擴弓器鄰近的組織區(qū)域其改建和修復(fù)程度更高。提示:靜磁場可能有促進(jìn)骨改建的效果。
[Abstract]:Objective:
Through animal experiments, using cone beam computed tomography (CBCT) and histological observation and comparison of the class organization, diamond coating adjustable integrated magnetic arch expansion appliance with the traditional screw expansion appliance expansion effect, in order to lay a foundation for the basic research of adjustable integrated magnetic arch expansion appliance and clinical practice.
Experimental methods:
1, measurement of mechanical properties of adjustable integrated magnetic expansion appliance
Select the N52 type high energy product of NdFeB magnets made adjustable integrated magnetic arch expansion appliance, magnet specifications: 11.5mm * 2.73mm * 2.3mm. curve using INSTRON universal dynamometer determination of magnetic arch expansion appliance scope is two times the force force 3mm.
2, animal experiment research
6 month old, early permanent dentition, no obvious maxillary occlusal defects, weighing about 8 kg of healthy male Beagle 18 rats were randomly divided into 3 groups: magnetic palatal expansion 6, expansion screw group 6, control group 6. In all canine maxillary first premolars (PM1), fourth premolars (PM4) buccal tip line, palatal sides by about 3mm vertical bone surface are respectively screwed into the self cured dentin nail as a location mark and preparation of magnetic palatal expansion group and screw group experimental canine maxillary arch expansion appliance model, according to the model were made in two groups sample. Magnetic palatal expansion group wore adjustable integrated magnetic arch expansion appliance, screw group wore traditional screw expansion appliance, at the intersection of the diagonal arch expansion device is arranged to form a sharp quadrilateral and the first molar mesiobuccal cusp on the cusp of the bilateral connection, while the control group was not wearing arch expansion appliance screw. Rotary expansion device group two times each week afterburner, rotating 3/4 ring, magnetic palatal expansion in the experimental group at two weeks by a mechanical limiting device, magnet spacing back to 0 loading time, 4 weeks after the end of expansion, all dogs were sacrificed three groups. Three groups of all samples respectively in experimental dogs wearing arch expansion appliance (T1) before and after the end of expansion (T2) of the head CBCT scan were recorded and measured T1, T2 width between maxillary canine, fourth premolars and first molar buccal cusp width, the mesiobuccal cusp width between the fourth premolar and first molar inclination and tilt (M1) of the first premolar area positioning nail and fourth premolar area positioning nail spacing, data statistics and analysis by statistical software SPSS. Three groups of all dogs were intercepted on both sides of the first premolar, maxillary expansion is adjacent to both sides of the central axis and the first molars between the palatal soft hard tissue, HE dyeing, The morphological changes of the middle suture of the palatine were observed under the optical microscope.
Experimental results:
1, mechanical properties of adjustable integrated magnetic arch appliance
After two forces of the magnetic force expansion appliance, a similar force distance curve, the force range of 10.38N-1.63N, and the distance of 0-3mm.
2, animal experiment results
2.1CBCT measurement results: magnetic palatal expansion group and screw group line measure project and PM4, increased M1 Inclination after the experiment, and the difference was significant, the control group after the experiment measuring items except PM4 inclination outside were also increased a little, and there was significant difference between the three groups. There is difference between the 22 measurements of T2-T1 phase comparison results show that the magnetic palatal expansion group and screw group all project difference was higher than control group, and the difference was significant; increase the amount of width of magnetic palatal expansion group canine than screw group, there were significant differences; and screw group PM1, PM4 district to increase the amount of nail positioning spacing than magnetic palatal expansion group, there were significant differences.
2.2 histological changes of palatal suture zone: magnetic palatal expansion group and screw group was significantly widened palatal suture, fibroblast, osteoblast proliferation, secretion is exuberant state, new bone deposition obvious seam edge, especially in the area of tissue adjacent to the magnetic arch expansion appliance the osteoblast proliferation, secretion of active network cells are star shaped, increased number of osteoclasts, shows a large number of vascular tissue remodeling and repair section, which is higher than the expansion screw group.
Conclusion:
1, the effect of the adjustable integrated magnetic expansion arch appliance in this study is remarkable
2, the bone effect of the adjustable integrated magnetic expansion arch appliance is smaller than the spiral expansion appliance, and the tooth effect is greater than the spiral expansion appliance.
3, the use of adjustable integrated magnetic expansion appliance and spiral expansion appliance can stimulate the bone remodeling in the middle of the palatine.
4, the adjacent tissue area of the adjustable integrated magnetic pantograph has a higher degree of reconstruction and repair. It is suggested that the static magnetic field may have the effect of promoting bone remodeling.
